Radical hypofractionated thoracic radiotherapy is the most commonly used radiotherapy schedule for inoperable non-small-cell lung cancer (NSCLC) in the United Kingdom, despite a lack of level I evidence to support its use.

To supplement existing published retrospective data with a mature data series and provide further evidence to support the use of this schedule in routine clinical practice.

Retrospective analysis of all inoperable NSCLC cases treated with radical hypofractionated radiotherapy with or without induction chemotherapy in the North Wales Cancer Treatment Centre between 2001 and 2011.

Of the 222 patients, 209 (94%) received 55 Gy in 20 fractions (#) and 13 (6%) received 52·5 Gy in 20#. Induction chemotherapy was administered in 121 (55%) cases. The median survival of 28·6 months (95% confidence interval 24·2–32·5) is comparable with previously published survival outcomes for this patient group.

The growing body of evidence for this schedule, confirming survival outcomes comparable with internationally accepted results, is sufficient to support its future use in inoperable NSCLC.

Success with acupuncture-like transcutaneous nerve stimulation (ALTENS) in the treatment of radiation-induced xerostomia has not been reported after an extended time.

We report a case of successful ALTENS therapy 5 years after radiation treatment.

The patient discontinued treatments in a second course due to complete resolution of symptoms.

This case report demonstrates a potential for offering ALTENS to those long suffering from radiation-induced xerostomia.

Sinonasal teratocarcinosarcoma (TCS) are highly aggressive and rare malignant tumours arising from the heterogeneous admixture of components of all the three germ cell layers. There are <60 cases that have been reported in the literature. In spite of aggressive therapy, the average survival is <3 years with multimodality therapy. In total, 70% of the patients who survived >1 year received regimen of combined surgery and adjuvant therapies and this suggests that aggressive therapeutic approaches may improve the treatment outcome.

We are reporting a case study of sinonasal TCS treated with initial surgery followed with concurrent chemoradiotherapy using intensity-modulated radiotherapy (IMRT) technique. The concurrent chemotherapy and adjuvant chemotherapy consisted of carboplatin and etoposide.

This aggressive treatment protocol of concurrent chemoradiation along with adjuvant chemotherapy is well tolerated and produced 5-year locoregional control and survival without any long-term morbidities.

Our treatment protocol was well tolerated and the outcome in this individual patient has been encouraging. This could justify a combined modality approach with post-operative simultaneous-integrated boost-IMRT and chemotherapy (concurrent and adjuvant) for future patient with sinonasal TCS.

Neuroblastoma is the most common extracranial solid tumour of infancy and accounts for about 6–10% of paediatric cancers. It has a biologically and clinically heterogeneous behaviour that ranges from spontaneous regression to cases of highly aggressive metastatic disease that could be unresponsive to standard therapy. In recent years, there have been several investigations into the development of various diagnostic, predictive and prognostic biomarkers towards personalised and targeted management of the disease.

This paper reports on the review of current clinical and emerging biomarkers used in risk assessment, screening for early detection and diagnosis, prognostication and monitoring of the response of treatment of neuroblastoma in paediatric patients.

Tumour markers can significantly improve diagnosis; however, the invasive, unpleasant and inconvenient nature of current tissue biopsies limits their applications, especially in paediatric patients. Therefore, the development of a non-invasive, reliable high accurate and personalised diagnostic tool capable of early detection and rapid response is the most promising step towards advanced cancer management from tumour diagnosis, therapy to patient monitoring and represents an important step towards the promise of precision, personalised and targeted medicine. Liquid biopsy assay with wide ranges of clinical applications is emerging to hold incredible potential for advancing cancer treatment and has greater promise for diagnostic purposes, identification and tracking of tumour-specific alterations during the course of the disease and to guide therapeutic decisions.

Choroidal metastases are the most frequent intraocular secondary tumours, with a prevalence of 2–7% according to the literature. Our aim was to review a clinical case of choroidal metastasis.

We present a case of a 63-year-old male patient diagnosed in 2018 with lung adenocarcinoma cT4N0M1. The patient had three metastases in the brain, which were successfully treated with radiosurgery (RS). The patient was treated with chemotherapy with pemetrexed–cisplatin schedule. Five months after diagnosis, the patient presented with decreased vision in the right eye. After ophthalmologic evaluation, he was diagnosed with a right choroidal metastasis, which was treated with external beam radiotherapy with 20 Gy in five fractions, resulting in improved visual acuity and a complete clinical and radiological response.

The patient took part in a clinical trial that continued with systemic chemotherapy. Twenty-two months after radiotherapy to the eye, the patient has good visual acuity without any side effects.

Choroidal metastasis treated with radiotherapy achieves good local control, with limited side effects, allowing an improvement in visual acuity and consequently, an improvement in the patient´s quality of life.

Monte Carlo calculation method is considered to be the most accurate method for dose calculation in radiotherapy. The purpose of this research is comparison between 6 MV Primus LINAC simulation output with commissioning data using EGSnrc and build a Monte Carlo geometry of 6 MV Primus LINAC as realistically as possible. The BEAMnrc and DOSXYZnrc (EGSnrc package) Monte Carlo model of the LINAC head was used as a benchmark.

In the first part, the BEAMnrc was used for the designing of the LINAC treatment head. In the second part, dose calculation and for the design of 3D dose file were produced by DOSXYZnrc. The simulated PDD and beam profile obtained were compared with that calculated using commissioning data. Good agreement was found between calculated PDD (1·1%) and beam profile using Monte Carlo simulation and commissioning data. After validation, TPR20,10, TMR and Sp values were calculated in five different field.

Good agreement was found between calculated values by using Monte Carlo simulation and commissioning data. Average differences for five field sizes in this approach is about 0·83% for Sp. for TPR20,10 differences for field sizes 10×10 cm2 is 0·29% and for TMR in five field sizes, the average value is ~1·6%.

In conclusion, the BEAMnrc and DOSXYZnrc codes package have very good accuracy in calculating dose distribution for 6 MV photon beam and it can be considered as a promising method for patient dose calculations and also the Monte Carlo model of primus linear accelerator built in this study can be used as method to calculate the dose distribution for cancer patients.

To measure and compare the skin doses received by treated left breast and contralateral breast (CB) during whole breast radiotherapy using five treatment techniques in an indigenously prepared wax breast phantom.

Computed tomography (CT) images of the breast phantom were used for treatment planning and comparison of skin dose calculated from treatment planning system (TPS) with measured dose. Planning target volume (PTV) and the CB were drawn arbitrarily on the CT images acquired for the breast phantom with 10 numbers of calibrated optically stimulated luminescent dosimeters (OSLDs) fixed on the surface of both breasts. The TPS calculated surface doses of PTV breast and CB for five treatment planning techniques, viz., conventional wedge (CW), irregular surface compensator-based (ISC), field-in-field (FiF), intensity-modulated radiotherapy (IMRT) and rapid arc (RA) techniques were obtained for comparison. The plans were executed in Clinac iX Linear Accelerator with the OSLDs fixed at the same locations on the phantom as in simulation. The TPS calculated mean dose at the surface of the treated left breast and CB was noted for the 10 OSLDs from dose-volume histogram (DVH) and compared with the measured dose. Also, the mean chamber dose at the centre of the left breast was noted from the DVH for comparing with ion chamber measured dose.

With reference to the results, it is seen that the dose to the CB is lowest in ISC technique and FiF technique and greatest in IMRT technique. The CW technique also delivered a dose comparable to IMRT to the CB of the phantom. The dose to the surface of PTV breast was highest and comparable in CW plans and FiF plans (68% and 67%) and lowest in IMRT and RA plans (50% each).

Analysis of the results shows that the FiF and ISC techniques are preferred while planning breast radiotherapy due to the reduced dose to the CB.

For palliative radiotherapy treatments, two types of simulation are available at our Centre: conventional (or 2D) and virtual. Each has its advantages: conventional simulation requires less preparation time whereas virtual simulation allows accurate visualisation and identification of target volumes in 3D. We propose a new approach where treatment field parameters are determined on diagnostic CT scans, and then reproduced with reference to a patient’s bony landmarks using conventional simulation. This combines the benefits of both methods. We argue that the slight differences in set-up between diagnostic and simulation CT scans will have little impact on the determination of the target volume in palliative treatment settings.

In total, 12 patients who had diagnostic scans done within a month before their virtual simulation were randomly selected. Both scans were retrieved retrospectively for the study. An independent radiation oncologist contoured the target volumes on both scans and their relative positions were compared by fusing the digitally reconstructed radiographs generated from the respective scans. A 2D Conformity Index (2DCI) was then calculated and tabulated for 0, 0.5, 1.0, 1.5 and 2 cm margins to evaluate the accuracy of this approach and determine the margins required to account for the inherent variability of this method. In addition, the deviation or offset of the centre of field (COF) was also measured and analysed.

The median 2DCI (0 margin) was 0.85. For margins of 0.5, 1.0, 1.5 and 2.0 cm, the median 2DCIs were 0.97, 1, 1 and 1, respectively. The median displacement of the COF in the superior–inferior direction was 0.7 cm and that in the right–left direction was 0.2 cm.

Our pilot study suggests that our approach is feasible. We recommend adding an additional margin of up to 0.5 cm (to the usual treatment margins) to ensure good coverage of the target volume when using this method.

To propose a new matching method for the supraclavicular (SC) and tangential fields on three-dimensional radiotherapy (3DRT) for postmastectomy radiotherapy (PMRT).

A method of matching coplanar field borders (CFB) between the tangential and SC fields was created in 3DRT. The collimator angle of the medial tangential field was calculated to coplanar the SC field. The proposed method performance was ultimately benchmarked using the half beam block (HBB) and traditional three-field monoisocenter (TTM) methods by dosimetric comparison. The decision score was then employed to clarify the performance among these methods.

The results show that the TTM method exhibited not only low doses on the organs at risk (OAR) but also on the matching fields. The CFB and HBB produced comparable results, but the ipsilateral lung yielded lesser amounts than the HBB. The decision score indicated a low performance level when using the TTM method, whereas the CBF method exhibited a slightly higher performance score than the HBB.

The CFB exhibited good performance in terms of the dose on OARs and at the matching fields. This method offers a comparable level of performance to the HBB. Thus, the CFB offers an alternative method of significant interest in PMRT.

It is already well-understood that patients requiring multiple hospital visits deal with several barriers. This paper considers a new methodology for determining the barrier that travel can cause, applying it to the mixed rural-city population of South-West Wales, calculating the travel burden for patients accessing radiotherapy. Travel burden could factor into conversations around optimisation of appointments and the impact of changes to treatment pathways.

Patient-specific travel data were calculated using Google Maps, for 1516 patients attending South-West Wales Cancer Centre for radiotherapy, modelled for 5-fraction and 15-fraction regimes.

28% of patients travelled for longer than 60 minutes. Moving to a 5-fraction treatment regime saves 20 one-way trips to the hospital, resulting in an average time saving of 15.9 hours for those travelling by car and 39.3 hours for those travelling by public transport. On average, this reduces carbon dioxide emissions by 91 kg per patient.

Implementation of a 5-fraction treatment regime has significantly reduced the travel burden for some patients receiving radiotherapy, as well as emissions related to travel. However, access to radiotherapy services in South-West Wales varies, with certain regions facing substantial travel burdens. Further research exploring other potential options to reduce travel burden is needed.

To evaluate the survival outcomes and toxicities experienced by non-metastatic head and neck cancer (HNC) patients receiving modulated radiotherapy (RT).

A total of 608 HNC patients treated consecutively from March 2010 to December 2014 with common subsites (oral cavity, oropharynx, hypopharynx, larynx and nasopharynx) of HNCs formed the study group. Eligible patients included those treated with radical or postoperative RT between March 2010 and December 2014. More than 90% patients received modulated RT [intensity-modulated radiotherapy (IMRT) or volumetric-modulated arc therapy (VMAT)] with concurrent chemotherapy as per stage guidelines. Demographic parameters and disease-related factors were analysed. Disease-free survival (DFS) was calculated from end date of RT till last follow-up or last date of disease control. Overall survival (OS) was calculated from date of registration to last follow-up date if alive. The primary endpoint was survival. The statistical analyses were performed using SPSS version 20.0 and Kaplan–Meier method was used for calculation survival.

Among the evaluable patients, the median age was 60 years (range: 16–93) with male preponderance (male:female – 513:95). Majority were squamous cell carcinoma 93·4% (568/608). The subsites treated were oral cavity 36·8% (224). oropharynx 26·4% (161), larynx 19·7% (120), hypopharynx 10% (62) and nasopharynx 6·4% (41). RT intent was radical in 63·5% (386) and postoperative in 36·5% (222), with 59·5% (362) receiving concurrent chemotherapy. At last follow-up, 348 (57·2%) patients were alive, 169 (27·7%) patients had succumbed to disease and 120 (24·6%) patients had recurrent disease. Out of 120 recurrent cases loco-regional recurrence, nodal recurrence and distant metastases were seen in 62 (51·7%), 25 (20·8%), 33 (27·5%), respectively. In the entire study cohort at 2 year OS and DFS was 80 and 79% whereas 3 years OS and DFS was 70 and 75%, respectively.

In our study, 2 years and 3 years OS and DFS rates are found comparable to the international data with acceptable toxicity profile with the use of modulated RT. It seems to be possible because of stringent departmental protocols and good medical physics support. Our data re-validates need and benefit of advanced RT techniques like IG-IMRT and VMAT for both postoperative and radical HNC treatment at the cost of minimal long-term side effects. Future stringent follow-up and quality of life issues are being considered in a prospective manner.

The main objective of this study was to compare dosimetric characterisation of high-dose-rate brachytherapy (HDR-BT) with external beam intensity-modulated radiation therapy (EX-IMRT) as a means of delivering boost dose.

Five HDR patients were selected for IMRT planning. Patients underwent ultrasound-guided catheter placement for HDR. Computed tomography (CT) images were obtained and imported into the Nucletron PLATO Brachytherapy system. The prostate, urethra, bladder and rectum were contoured on axial slices. The dose was calculated and optimised by graphical optimisation. The CT images of these structures were exported from the PLATO to Eclipse workstation for IMRT planning. For each patient, the dose–volume histogram (DVH) of HDR and IMRT plans were generated, drawn on the same scale and compared.

The dose distribution in HDR plans was non-uniform and conformed peripherally inside the planned target volume (PTV). A small volume of the prostate received a very high dose from HDR.In IMRT plans, a uniform dose distribution was observed. The DVH curves for PTV dropped sharply and reached to a zero volume of the prostate at about 6·4 Gy. In HDR plans, the DVH curves for PTV showed a long tail up to a very high dose. About 10% of the prostate received about 13·3 Gy, which is 222% of the prescribed dose (6 Gy) in HDR plans. In contrast, the same volume in IMRT plans received <6 Gy (100%). The average dose for V90 was about 6·3 Gy for HDR and 5·8 Gy for IMRT plans. At a prostate volume of V100 level, the average dose in all plans was 5·0 Gy from HDR and 5·4 Gy from IMRT plans. In HDR plan, the V100 dose for urethra varied from 0·6 to 3·0 Gy (average 1·8 Gy). The range in IMRT plans varied from 3·6 to 6 Gy with an average of 4·7 Gy. At V90 level, the dose range in HDR and IMRT plans varied from 2·5 to 4·7 Gy (average 3·8 Gy) and 4·8 to 5·4 Gy (average 5·3 Gy), respectively. In general, the dose to the bladder and rectum was comparatively lower in HDR than in IMRT plans.

HDR brachytherapy may reduce normal tissue toxicities in prostate boost treatments, even though the dose homogeneity inside the PTV is far worse than in IMRT treatments. Another advantage of HDR over IMRT is that the organ motion is not a significant concern as in IMRT.
