Intraoperative radiotherapy is a method of choice to deliver a critical radiation dose to the tumour bed immediately after surgical excision.

The purpose of this work is to check the dose delivered to the patients during intraoperative electron beam radiation therapy (IOERT) in the conservative treatment of breast cancer, by means of reference dose measurement using radiochromic (EBT-2) films.

Ninety patients with early-stage breast cancer underwent exclusive IOERT to the tumour bed using a LIAC linear accelerator. Absolute dose measurements were done with film pieces. After irradiation, the pixel values of the films were obtained via MATLAB and ImageJ softwares. Calibration curve was also used for calculating net optical density. Expected dose was compared to the patient delivered dose.

The mean deviation of the delivered dose from the expected one was 2·56% that is well in the accepted criteria. Only in one case, there was a larger deviation due to barometer miscalibration.

EBT-2 film response is independent from dose-per-pulse and as it was shown in this study it can be robustly used during breast IOERT for dosimetric and also positioning verifications.

Increasing usage of magnetic resonance imaging (MRI) in radiotherapy (RT) and the advent of MRI-based image-guided radiotherapy (IGRT) suggests a need for additional training within the RT profession. This critical review aimed to identify potential gaps in knowledge by evaluating the current skill base in MRI among therapeutic radiographers as evidenced by published research.

Papers related to MRI usage were retrieved. Topic areas included outlining, planning and IGRT; diagnosis, follow-up and staging-related papers were excluded. After selection and further text analysis, papers were grouped by tumour site and year of publication.

The literature search and filtering resulted in a total of 123 papers, of which 66 were related to ‘outlining’, 37 to ‘planning’ and 20 to ‘IGRT’. The main sites of existing MRI expertise in RT were brain, central nervous system, prostate, and head and neck tumours. Expertise was clearly related to regions where MRI offered improved soft-tissue contrast. MRI studies within RT have been published from 2007 onwards at a steadily increasing rate.

Current use of MRI in RT is mainly restricted to sites where MRI offers a considerable imaging advantage over computed tomography. Given the changing use of MRI for image guidance, emerging therapeutic radiographers will require training in MRI interpretation across a wider range of anatomical regions.

Recurrence is frequent in surgically treated non-functioning pituitary adenomas (NFPA). The use of radiation therapy (RT) to prevent recurrence has to be weighted against the potential side effects, particularly, hypopituitarism. Our objective was to evaluate the efficacy and safety of RT in postoperative patients with NFPA with adenoma remnant.

The 3- and 5-year outcome of 51 patients with NFPA with a remnant after surgery that received RT (cases) was compared with that of 61 subjects who did not receive RT (controls). Cases and controls were matched for postoperative remnant size, cavernous sinus invasion, age and gender.

Tumour volume decreased in the radiated group from a median of 1,601 mm3 to 816 mm3 after 5 years of follow-up (p = 0·01, 50% tumour volume reduction). In the non-radiated controls median tumour volume decreased at 3 years but increased again after 5 years (baseline 1,415 mm3, 5 years 1,204 mm3, p = 0·93). Recurrence rate was 4% for the radiated group and 29% for the controls (OR 0·10, 95% CI 0·01–0·04, p = 0·02). Although pituitary hormone deficiencies at baseline were more prevalent in the radiated group, after 5 years, both groups showed a significant worsening of pituitary function. No RT-related side effects were recorded.

Postoperative RT is effective in preventing tumour regrowth in NFPA patients with postoperative remnants. The fact that hypopituitarism is highly prevalent even in non-radiated patients should allow a more generalised use of this treatment modality.

At The Radiation Medicine Program described, the entire radiation therapy (RT) workflow was previously conducted through the use of two electronic programs. It duplicated workflow and created a situation where it was difficult to measure the RT process. Recent enhancements to the electronic medical record facilitated the consolidation of RT planning and treatment workflows into one electronic system.

This report will describe the clinical implementation of electronic Radiation Oncology (RO) Care Plans at a Regional Cancer Centre, and how they can be applied as a foundation for RT process improvements.

A total of 51 Care Plans and 95 IQ Scripts were successfully implemented. The benefits of RO Care Plans include a more streamlined process, removed ambiguity, improved communication, standardised workflow and automation of tasks. In addition, multiple performance indicators can be obtained from the RO Care Plans, such as caseload reports, workflow reports and a ‘white board’.

The implementation of RO Care Plans serves as a foundation for data-driven process improvement at a local Regional Cancer Centre.

The complexity associated with the treatment planning and delivery of stereotactic radiosurgery (SRS) or stereotactic radiotherapy (SRT) volumetric modulated arc therapy (VMAT) plans which employs continuous dynamic modulation of dose rate, field aperture and gantry speed necessitates diligent pre-treatment patient-specific quality assurance (QA). Numerous techniques for pre-treatment VMAT treatment plans QA are currently available with the aid of several different devices including the electronic portal imager (EPID). Although several studies have provided recommendations for gamma criteria for VMAT pre-treatment QA, there are no specifics for SRS/SRT VMAT QA. Thus, we conducted a study to evaluate intracranial SRS/SRT VMAT QA to determine clinical action levels for gamma criteria based on the institutional estimated means and standard deviations.

We conducted a retrospective analysis of 118 EPID patient-specific pre-treatment QA dosimetric measurements of 47 brain SRS/SRT VMAT treatment plans using the integrated Varian solution (RapidArcTM planning, EPID and Portal dosimetry system) for planning, delivery and EPID QA analysis. We evaluated the maximum gamma (γmax), average gamma (γave) and percentage gamma passing rate (%GP) for different distance-to-agreement/dose difference (DTA/DD) criteria and low-dose thresholds.

The gamma index analysis shows that for patient-specific SRS/SRT VMAT QA with the portal dosimetry, the mean %GP is ≥98% for 2–3 mm/1–3% and Field+0%, +5% and +10% low-dose thresholds. When applying stricter spatial criteria of 1 mm, the mean %GP is >90% for DD of 2–3% and ≥88% for DD of 1%. The mean γmax ranges: 1·32 ± 1·33–2·63 ± 2·35 for 3 mm/1–3%, 1·57 ± 1·36–2·87 ± 2·29 for 2 mm/1–3% and 2·36 ± 1·83–3·58 ± 2·23 for 1 mm/1–3%. Similarly the mean γave ranges: 0·16 ± 0·06–0·19 ± 0·07 for 3 mm/1–3%, 0·21 ± 0·08–0·27 ± 0·10 for 2 mm/1–3% and 0·34 ± 0·14–0·49 ± 0·17 for 1 mm/1–3%. The mean γmax and mean γave increase with increased DTA and increased DD for all low-dose thresholds.

The establishment of gamma criteria local action levels for SRS/SRT VMAT pre-treatment QA based on institutional resources is imperative as a useful tool for standardising the evaluation of EPID-based patient-specific SRS/SRT VMAT QA. Our data suggest that for intracranial SRS/SRT VMAT QA measured with the EPID, a stricter gamma criterion of 1 mm/2% or 1 mm/3% with ≥90% %GP could be used while still maintaining an in-control QA process with no extra burden on resources and time constraints.

Radiotherapy to the head and neck region may cause considerable radiotherapy-induced changes in the surrounding tissues. These changes are oral mucositis, hyposalivation, dental caries, osteoradionecrosis, trismus and overall impact on patients’ quality of life. Tooth-coloured synthetic materials, unlike metallic restoration, did not influence radiation dose distribution. However, their exposure to a gamma radiation therapeutic dose during treatment might cause structural and compositional changes that alter their mechanical and physical properties.

This study intends to evaluate the effect of Co 60 gamma rays on shear bond strength and marginal adaptation of already restored tooth surfaces, to help in material selection before the onset of radiotherapy.

Hundred freshly extracted human permanent mandibular molar teeth collected and stored in a 0·2% thymol solution for disinfection and were randomly divided into two groups of 50 each, to be tested for the shear bond strength of restoration to dentin and the marginal gap at tooth–restoration interface, respectively.

ANOVA showed a significant effect of both radiotherapy (F = 40·33, p < 0·001) and restorations (134·00, p < 0·001) on the marginal gap at the interface. In the without radiotherapy group, the mean marginal gap was least in Group Z250, and in with radiation, Bulk Fill has the least mean marginal gap. The mean shear bond strength was comparatively higher for all restorations without radiation than with radiation (p < 0·001).

Gamma radiation affects the physical or mechanical properties of tooth structure and the tooth restorative interface. Composites seem to be good restorative material when placed before the onset of radiotherapy in head and neck cancer patients.

This paper is a methodological reflection on the use of interpretative phenomenological analysis (IPA) utilised in the context of a qualitative research project that explored perceptions and experiences of the journey to radiographer advanced practice.

A two-phase qualitative research explored the perceptions and experiences. Phase 1 reviewed reflective diaries (n = 12) kept during the educational phase of the practitioner journeys. Phase 2 included one-to-one, semi-structured interviews (n = 6) which were recorded, transcribed verbatim and reviewed using the IPA six-stage thematic analysis for practitioners embedded in the advanced practice role.

Key themes arising from reflective diary analysis informed the interview content; and following interview transcription, data immersion and IPA, 12 emergent sub-themes generated 3 superordinate themes.

Theoretical perspectives and application of the methodology are discussed. The phenomenological and interpretative qualities of IPA have the potential to provide unique and valuable insights into lived experiences of individuals. It is hoped that this researchers’ reflections are transferrable for those interested in employing a qualitative methodology for radiotherapy and oncology research.

Therapeutic radiographers work within rapidly changing environments from technological, treatment and care perspectives. With continued development and change, the impact of research utilising an IPA methodology may allow exploration of perceptions and experiences from a range of key stakeholders with the potential to increase the research base.

A comparative study was performed about the plan parameters and quality indices between volumetric arc therapy (VMAT) and intensity-modulated radiotherapy (IMRT) for the treatment of high-risk prostate cancer patients. The aim of this retrospective study was to compare the two methods of external beam radiotherapy IMRT and VMAT in terms of plan quality and efficacy.

Fifteen high-risk prostate patients were planned for radiotherapy using 6 MV photon. Three dose levels were contoured having Planning Tumour Volume 1 (PTV1 = 48 Gy), Planning Tumour Volume 2 (PTV2 = 57.6 Gy) and Planning Tumour Volume 3 (PTV3 = 60 Gy). Setup margins were given using the CHIP trial method. The prescribed PTV3 dose was 60 Gy in 20 fractions which is biologically equivalent to 74 Gy in 37 fractions using α/β = 3. In case of IMRT, seven fixed beam angles 30, 60, 105, 180, 255, 300 and 330 were used and the dose was optimised using the sliding window method. In case of rapid arc technique, one or two full arcs were used for dose optimisation while keeping all the dose constraints and other planning parameters same used in IMRT. The plan evaluation parameters and Organ at risks (OARs) doses were calculated using a dose volume histogram (DVH).

The average D2, D5, D95 and PTVmean for PTV3 were 61.22, 61.13, 58.12, 60.00 Gy and 62.41 62.24 59.53 61.12 Gy for IMRT and VMAT, respectively. The averages V60 for bladder and V30 for rectum were 22.81, 25 and 67, 65% for IMRT and VMAT, respectively. The average homogeneity index (HI), conformity index (CI) and gradient index (GI) were 1.04, 1.4833, 14.79 and 1.04, 1.704, 7.89 for IMRT and VMAT, respectively.

VMAT takes less dose-delivery time and lesser number of monitoring units than IMRT, thus it compensates the intrafractional movements during dose delivery. The Dose GI in VMAT was much better than IMRT. This indicates sharper dose fall off near the normal tissue. No other major differences were observed in terms of plan evaluation parameters between IMRT and VMAT techniques. So, we conclude that VMAT technique is more efficient than IMRT in terms of plan quality and dose delivery.

Dose distribution index (DDI) is a treatment planning evaluation parameter, reflecting dosimetric information of target coverage that can help to spare organs at risk (OARs) and remaining volume at risk (RVR). The index has been used to evaluate and compare prostate volumetric modulated arc therapy (VMAT) plans using two different plan optimisers, namely photon optimisation (PO) and its predecessor, progressive resolution optimisation (PRO).

Twenty prostate VMAT treatment plans were created using the PO and PRO in this retrospective study. The 6 MV photon beams and a dose prescription of 78 Gy/39 fractions were used in plans with the same dose–volume criteria for plan optimisation. Dose–volume histograms (DVHs) of the planning target volume (PTV), as well as of OARs such as the rectum, bladder, left and right femur were determined in each plan. DDIs were calculated and compared for plans created by the PO and PRO based on DVHs of the PTV and all OARs.

The mean DDI values were 0·784 and 0·810 for prostate VMAT plans created by the PO and PRO, respectively. It was found that the DDI of the PRO plan was about 3·3% larger than the PO plan, which means that the dose distribution of the target coverage and sparing of OARs in the PRO plan was slightly better. Changing the weighting factors in different OARs would vary the DDI value by ∼7%. However, for plan comparison based on the same set of dose–volume criteria, the effect of weighting factor can be neglected because they were the same in the PO and PRO.

Based on the very similar DDI values calculated from the PO and PRO plans, with the DDI value in the PRO plan slightly larger than that of the PO, it may be concluded that the PRO can create a prostate VMAT plan with slightly better dose distribution regarding the target coverage and sparing of OARs. Moreover, we found that the DDI is a simple and comprehensive dose–volume parameter for plan evaluation considering the target, OARs and RVR.

In those receiving radiotherapy for pelvic cancers, up to 80% develop gastrointestinal symptoms, with dietary interventions recommended to reduce these symptoms. However, research outlining the current dietary support provided to patients undergoing radiotherapy for pelvic cancer is lacking.

To identify the gastrointestinal symptoms experienced by those undergoing pelvic radiotherapy and to identify the dietary support provided to these patients.

A service evaluation was undertaken in one NHS Trust hospital whereby patients undergoing radical pelvic radiotherapy during a 15-week recruitment period were invited to complete an anonymous questionnaire. Participants were recruited using purposive sampling and the data were analysed descriptively using SPSS.

In total, 31 patients responded achieving a response rate of 48%. The most frequent reported gastrointestinal symptoms were gas and flatulence followed by diarrhoea, nausea and abdominal pain. The main dietary changes implemented by the respondents and recommended by health care professionals included reducing fibre intake, reducing certain vegetables, reducing caffeine and increasing water.

The results illustrate the impact of gastrointestinal side effects on patients’ dietary intake. The results highlight that nutritional guidance need to be standardised, especially for the management of diarrhoea and gas and flatulence as these were the most common occurring side effects. With radiographers most frequently giving nutritional advice they must be provided with guidance to support those undergoing pelvic radiotherapy.

Radiation therapy (RT) remains a common and effective treatment modality for patients with locally advanced prostate cancer. Technological advancements over the past decade have resulted in the introduction of intensity-modulated radiation therapy (IMRT) planning and delivery techniques that maximise the dose of radiation delivered to the prostate while sparing organs at risk (OAR). A more recent and evolving IMRT technique, called volumetric-modulated arc therapy (VMAT), involves a continuous irradiation at a constant or variable dose rate when the gantry rotates around the prostate using one or more arcs.

This paper reports on a dosimetric evaluation of our implementation of VMAT technique for prostate cancer treatment. A retrospective analysis of VMAT plans was performed for 300 prostate cancer patients treated during the period of January 2013 to December 2014. Two prescription cohorts of patients treated to a dose of 78 Gy in 39 fractions as the primary radiation therapy treatment (XRT) and 66 Gy in 33 fractions as a post-op or salvage XRT were considered.

The mean and maximal doses, dose inhomogeneities and conformity indexes for the planning target volumes were evaluated for each prescription cohort of patients. Similarly, the doses to OAR such as rectum, bladder and femoral heads were also assessed for various dose levels.

This study shows that highly conformal radiation dose distribution for the treatment of prostate cancer is achievable with the VMAT technique. It provides evidence to support the adoption of such conformal technology in many disease sites such as the prostate. We believe that our experience reported here could help form the foundation for individual institutions to evaluate and develop the most suitable planning criteria tailored to their own needs and priority. This endeavour hopefully will provide further improvement in the planning process and, therefore, help achieve an effective and efficient delivery of radiotherapy for prostate cancer.

Lung tumours, especially those in the lower lobes, can move a lot during respiration; this motion needs to be accounted for during radiotherapy. In cases where 4D CT simulation scans are not performed, the current protocol at our centre is to apply a generic (internal motion + setup) margin of 0·70 cm in the axial plane and 1·20 cm in the longitudinal plane to all lung tumours, regardless of location. We analyse the tumour motions of a cohort of our local patients and categorise them into different locations in the lung. We seek to assess the adequacy of the current margins and to derive a more accurate set of standard margins which are specific for lung tumour locations.

All cases of lung tumours treated with stereotactic ablative radiotherapy between 2012 and 2016 were identified retrospectively and 4D CT scan data analysed. These tumours were grouped into the following locations: upper zone (UZ), middle zone (MZ) and lower zone (LZ). The treatment planning system was used to generate the displacements of the centre of mass of the tumours in the right–left, anterior–posterior and superior–inferior axes; these were compared with the current generic margins. Median displacements were calculated for each axis in each location. New planning target volume (PTV) margins were derived by summing the median displacement, median absolute deviation (MAD) and 0·5 cm (for setup error).

Sixty-three cases were eligible for analyses. Motion in the superior–inferior direction was the greatest for all tumour locations, ranging from a median of 0·17 cm (MAD 0·12 cm) in UZ to 0·77 cm (MAD 0·27 cm) in LZ. Median tumour displacements in the anterior–posterior and right–left axes were similar for all locations, <0·30 and 0·20 cm, respectively. The current generic margins were adequate for only one-third of the cases in this study. A new PTV margin of 2·10 cm in the superior–inferior axis may be required for LZ tumours, while an additional 1–2 mm should be added to the current radial margins.

The current generic margins are inadequate for the majority of cases. Tumour motion is the greatest in LZ in the superior–inferior axis. Motion mitigation strategies are essential for large LZ tumours.

This is a prospective study to evaluate the dosimetric benefits of treatment plan adaptation for patients who had undergone repeat computed tomography (ReCT)and re-planning due to treatment-induced anatomical changes during radiotherapy.

This study involved five head and neck cancer patients who had their treatment plan modified, based on weekly thrice imaging protocol. Impact of mid-course imaging was assessed in patients using ReCT and cone beam computed tomography (CBCT)-based dose verification. Patients were imaged, apart from their initial CT, during the course of their radiation therapy with a ReCT and on board imager CBCT (Varian Medical Systems Inc., Palo Alto, CA, USA). Each CBCT/CT series was rigidly registered to the initial CT in the treatment planning system Eclipse (Varian Medical Systems Inc.) using bony landmarks. The structures were copied to the current CBCT/CT series and, where needed, manually edited slicewise. The dose distribution from the treatment plan was viewed as of the current anatomy by applying the treatment plan the CBCT/CT series, and studying the corresponding dose–volume histograms for organs at risk doses.

The reduction of parotid volumes due to weight loss was observed in all patients, which means an increase in predicted mean doses of parotid when initial CT plan was re-calculated on ReCT and CBCT (Table 1). This explains the necessity of adaptive planning. The predicted mean dose of parotid glands was increased and constraints to spinal cord and skin were exceeded, so re-planning was performed.

The CBCT is a useful tool to view anatomic changes in patients and get an estimate of their impact on dose distribution. Re-planning based on imaging in head and neck patients during the course of radiotherapy is mandatory to reduce side effects.

Mastectomy is a treatment option for patients diagnosed with breast cancer. There is very limited research into the psychosocial impact of mastectomy on female breast cancer patients, especially in the sub-Saharan African setting which has unique cultural norms. The study aimed at assessing the psychosocial impact of mastectomy on female breast cancer patients attending a radiotherapy/oncology centre in Ghana.

A cross-sectional design was used to undertake this study. A total of 80 female mastectomy breast cancer patients participated in this study. A semi-structured questionnaire was used for data collection over a 5-month period, January to May 2018. The data collected were analysed with Statistical Package for Social Science (SPSS) version 22.

Most of the participants were adversely affected psychologically and emotionally by mastectomy. In particular, many (56·7%) agreed that they felt less feminine, and 71% reported they experienced psychological distress as a result of their mastectomy, while 63% of them reported loss of self-confidence. A large proportion of them (51, 63·8%) agreed that their lifestyles had changed following mastectomy, while 58% of them admitted that they felt treated as outcasts by society, and 75% resorted to the use of breast prostheses to reduce attention.

The study revealed that mastectomy for breast cancer patients had a negative impact on their psychological, emotional and social well-being. The availability of affordable breast prostheses, involvement of clinical psychologists in the care of post-mastectomy women, provision of emotional, psychological and even financial support could alleviate the psychosocial impact of affected women.

The aim of this study was to investigate the extent to which lung stereotactic body radiotherapy (SBRT) treatment plans can be improved by replacing conventional flattening filter (FF) beams with flattening filter-free (FFF) beams.

We selected 15 patients who had received SBRT with conventional 6-MV photon beams for early-stage lung cancer. We imported the patients’ treatment plans into the Eclipse 13·6 treatment planning system, in which we configured the AAA dose calculation model using representative beam data for a TrueBeam accelerator operated in 6-MV FFF mode. We then created new treatment plans by replacing the conventional FF beams in the original plans with FFF beams.

The FFF plans had better target coverage than the original FF plans did. For the planning target volume, FFF plans significantly improved the D98, D95, D90, homogeneity index and uncomplicated tumour control probability. In most cases, the doses to organs at risk were lower in FFF plans. FFF plans significantly reduced the mean lung dose, V10, V20, V30, and normal tissue complication probability for the total lung and improved the dosimetric indices for the ipsilateral lung. For most patients, FFF beams achieved lower maximum doses to the oesophagus, heart and the spinal cord, and a lower chest wall V30.

Compared with FF beams, FFF beams achieved lower doses to organs at risk, especially the lung, without compromising tumour coverage; in fact, FFF beams improved coverage in most cases. Thus, replacing FF beams with FFF beams can achieve a better therapeutic ratio.

Cancer during pregnancy has an incidence of 1/1,000, and when diagnosed, the most common ones are breast cancer (1/10,000), Hodgkin’s lymphoma (1/6,000), and head and neck cancer (1/10,000). If a diagnosis is made during pregnancy, the treatment cannot wait until delivery, and there is concern about the effects that radiotherapy may have on the foetus. The multidisciplinary group has to assess and ethically make decisions with regard to the mother and foetus.

A 35-year-old female, a carrier of Behcet’s disease, underwent 5 years of treatment with hydroxychloroquine, prednisone and low-molecular-weight heparin (the patient being a carrier of Behcet’s disease, there is a high risk for cancer of the oral cavity and oropharynx with an HR of 2·11, so the cancer could be related to the tonsil). The patient’s oncological situation started on December 2017 with a volume increase in preauricular, parotid and right mandibular angle, with a progressive growth. At this time, the foetus was of 17·5 weeks of gestation. An oral cavity tumour that invaded the right retromolar triangle was observed, and upon biopsy, a basaloid squamous cell carcinoma was diagnosed.

Radiotherapy treatment was started at 22 weeks of gestation; intensity-modulated radiation therapy (IMRT) was planned with a dose of 69·96 Gy to the primary tumour and 59·4 Gy to ganglion levels II, III and IV, bilaterally in 33 fractions. At fraction 27 a significant decrease in tumour volume was noted, so adaptive radiotherapy was performed to complete the treatment. Currently the patient has no clinical evidence of tumour pathology.

The risk of radiation exposure in pregnant women (after 20 weeks of gestation), being treated for cancers of the tonsil, reaching the foetus is minimal, with a reduced risk of a few or no effects.

Radiotherapy in tonsil cancer has been shown to be effective in combination with chemotherapy for local control of the disease. In the case of this pregnant patient, radiotherapy, as the only modality, provided local control and little exposure of radiation to the foetus.
