Millions of children who were born during the first decade after the Islamic revolution in Iran are now reaching the age of marriage and childbearing. Short spacing between marriage and the birth of the first child has the potential to cause an excessive and costly increase in the growth of population in Iran. Research into the motivations for the birth of first child among newly married couples can create a knowledge base that will enable health centres to help these couples make better decisions about the timing of their first pregnancy. Using a consecutive sampling technique and administering Miller’s Childbearing Questionnaire, data were gathered regarding the childbearing motivations and desires of 300 couples who had been referred to the Shiraz Health Center for premarital counselling. The Childbearing Questionnaire, with some minor modifications, was found to be a valid and reliable instrument for measuring the childbearing motivations of newly married couples of Shiraz County, Fars Province, Iran. The utility of these findings for counselling in health centres is discussed. Based on the results, a longitudinal study is being designed that will allow the development of models for predicting the time of first pregnancy after marriage.

Obesity is pandemic, but no consensus has yet emerged regarding appropriate tools for measuring it. Medical research based on populations in the developed world has largely dismissed body mass index (BMI) because it is a weak predictor of some health outcomes. In contrast, social science research still relies on it for its simplicity and ready availability in surveys. This paper uses consistent definitions and measures to select sufficiently good predictors of health and economic outcomes from among the anthropometrics that are considered alternatives to BMI. The results from the Indonesian Family Life Survey indicate that BMI and waist circumference are better predictors than waist-to-height ratio and waist-to-hip ratio. This paper argues that given its advantages, BMI is an adequate measure of obesity for Indonesia and possibly for the developing world. Further, if BMI is to be replaced, waist circumference is preferable to other anthropometrics.

The prevalence and determinants of primary caesarean section in Jamaica were estimated from a survey of women aged 14–49 years. Among 2328 women reporting 2395 live hospital births during the period January 1984 to May 1989, the prevalence of caesarean section was 4·1%. Repeat caesarean sections accounted for 1·3% of the hospital births during that period. Of the medical complications studied, prolonged labour and/or cephalopelvic disproportion carried the highest risks of primary caesarean section, followed by breech presentation, maternal diabetes, a high birth-weight baby, maternal hypertension, and a low birth-weight baby. The risk of primary caesarean section increased with maternal age, decreased with parity, was higher for urban than for rural residents, and was higher for births in private versus government hospitals.

Beliefs about child illness were investigated using semi-structured interviews with mothers and providers in four rural Guatemalan communities. The two most common forms of child illness in Guatemala – diarrhoea and respiratory disease – were focused upon. These illnesses are particularly difficult to prevent and treat, especially with the rudimentary health services available in rural areas of developing countries. Comparisons with other ethnographic studies in Guatemala suggest that some traditional models of illness causation identified in these earlier investigations are relatively unimportant in the communities studied here. This finding, in conjunction with frequent responses related to hygiene and water, suggests that traditional explanations may be co-existing with biomedical views of illness causation to a greater degree today than in the past.

The paper looks at the limitations and strengths of using the A-cope questionnaire for measuring strategies for coping with prolonged conflict by Palestinian young people in Gaza. The scale was administered to young people between the ages of 8 and 17. The results show some gender differences in coping strategies. However, some items on the subscales are not relevant for Muslim societies or societies in situations of prolonged conflict. The authors suggest that combining an anthropological contextual perspective and qualitative data with psychological instruments is an effective way of addressing the limitations of using a single quantitative method of assessment in non-Western complex social and cultural settings.

Women who bear a child in their teens differ in their childhood, adolescent and adult experiences from their peers who delay childbearing until later ages. Within the group of teenage mothers the background characteristics associated with and the consequences of pregnancy status at first birth are discussed.

Summary. With the rapid pace of the nutrition transition worldwide, understanding influences of child feeding practices within a context characterized by the co-existence of overweight and undernutrition in the same population has increasing importance. This qualitative study describes Brazilian mothers’ child feeding practices and their perceptions of their association with child weight status and explores the role of socioeconomic, cultural and organizational factors on these relationships. Forty-one women enrolled in the Family Health/Community Health Workers Programme were selected from rural, urban, coastal and indigenous areas in Ceara State, north-east Brazil, to participate in four focus group discussions. Content analysis identified fourteen emergent themes showing mothers’ child feeding practices in this setting were influenced by economic resources, mothers’ immediate social support networks (e.g. neighbours and family members) and participation in nutrition assistance programmes. Child malnutrition was the most common nutritional concern; nevertheless, mothers were aware of the negative health consequences of obesity but misunderstood its causes (e.g. foods filled with fat would make a person fat; others thought that birth control pills and stimulants given to children were causes of obesity); several reported their own struggles with overweight. Food assistance programmes emerged as an important influence on children’s dietary adequacy, especially among mothers describing dire economic situations. The findings have implications for targeting food assistance as well as health and nutrition education strategies in low-income families undergoing the nutrition transition in north-east Brazil.

Utilization of antenatal care (ANC) services, as part of reproductive health care, presents a lifesaving chance for health promotion and the early diagnosis and treatment of illnesses throughout pregnancy. This study examines the factors associated with the number and timing of ANC visits among married women in Cameroon using data from the 2018 Cameroon Demographic and Health Survey. The outcome variables were number of ANC visits, categorized as <8 visits or ≥8 visits, and the timing of first ANC visit, categorized as ≤3 months (early) or >3 months (late) (as per the new 2016 WHO recommendations). Descriptive statistics and binary logistic regression were used to analyse the data. Crude odds ratios (cOR) and adjusted odds ratios (aOR) and p-values with significance at <0.05 were used to interpret the results. The proportions of women who had ≥8 ANC visits and first ANC visit at ≤3 months gestation were 6.3% and 35.6% respectively. Women aged 35–39 at childbirth (aOR=3.99, 95% CI=1.30–12.23), middle wealth quintile women (aOR=3.22, 95% CI=1.01–10.27), women whose husbands had secondary (aOR=7.00, 95% CI=2.26–21.71) or higher (aOR=16.93, 95% CI=4.91–58.34) education were more likely to have ≥8 ANC visits. Early timing of first ANC visit was low among women with birth order 3–4 (aOR=0.63, 95% CI=0.46–0.85). Conversely, the likelihood of having early ANC visits was high among women whose pregnancies were intended (aOR=1.32, 95% CI=1.01–1.74), the richest women (aOR=3.89, 95% CI=2.30–6.57) and women whose husbands had secondary (aOR=2.41, 95% CI=1.70–3.64) or higher (aOR=3.12, 95% CI=2.40–7.46) education. The study highlights that age at childbirth, wealth, husband’s educational attainment, birth order and pregnancy intention could influence the utilization of ANC services among married women in Cameroon. Hence, to improve attendance and early initiation of ANC, interventions should be targeted at empowering women financially and removing all financial barriers associated with accessing ANC, improving ANC education among women and encouraging male involvement in ANC education.

The human menstrual cycle is characterized by a series of dynamic hormonal relationships that result in the regular maturation of ovarian follicles, ovulation and corpus luteum formation. As methodology has changed and improved, it has become possible to examine these events in greater detail. The simple analysis of menstrual cycle length, when done with care in a large number of women over long time spans, has revealed much about the fundamental biological rhythm of the menstrual cycle. Of particular interest are the predictable changes in cycle length and variability that occur at different stages of reproductive life (Treloar et al., 1967; Vollman, 1977). Basal body temperature records add another dimension to the analyses of menstrual cycle length because they permit estimation of the length of the pre- and post-ovulatory phases of the menstrual cycle and provide some index of the integrity of corpus luteum function.

Japanese and British 9-year-old children were compared on the standard progressive matrices and twelve reaction time parameters providing measures of simple and complex decision times, movement times and variabilities. The mean of the Japanese children on the progressive matrices exceeded that of the British children by 0·65 SD units and on the decision times component of reaction times by 0·50 SD units, suggesting that the high Japanese mean on psychometric intelligence is largely explicable in terms of the more efficient processing of information at the neurological level. Japanese children also showed faster movement times but, contrary to expectation, had greater variabilities than British children.

According to official census returns from Roman Egypt (first to third centuries CE) preserved on papyrus, 23·5% of all documented marriages in the Arsinoites district in the Fayum (n=102) were between brothers and sisters. In the second century CE, the rates were 37% in the city of Arsinoe and 18·9% in the surrounding villages. Documented pedigrees suggest a minimum mean level of inbreeding equivalent to a coefficient of inbreeding of 0·0975 in second century CE Arsinoe. Undocumented sources of inbreeding and an estimate based on the frequency of close-kin unions (corrected downwards to 30% for Arsinoe) indicate a mean coefficient of inbreeding of F=0·15-0·20 in Arsinoe and of F=0·10-0·15 in the villages at the end of the second century CE. These values are several times as high as any other documented levels of inbreeding. A schematic estimate of inbreeding depression in the offspring of full sibling couples indicates that fertility in these families had to be 20-50% above average to attain reproduction at replacement level. In the absence of information on the amount of genetic load in this population, this estimate may be too high

This Study, the first of a series on consanguinity in the Archbishopric of Toledo from 1900 to 1979, analyses in detail the types of consanguineous mating in a population of considerable size, some 650,000 persons, in central spain. during the period covered, very important transformations occurred in Spanish society. The data relate to 21,464 consanguineous marriages. They were divided into two groups, according to the premarital migration of the couple. Significant differences in the frequencies of the types of consanguineous matings, and their secular evolution, are found for the two groups. The expected trend in inbreeding in the total area shows two different tendencies: constancy till the end of the 1950s, followed by a fast decline during the last two decades.

Reproductive strategies are related to ecological constraints. This paper examines data on early birth spacing in a scheduled caste, Bengali-speaking, non-contracepting population of the Karimganj district of southern Assam, India, taking an evolutionary ecological perspective. It is found that on average birth intervals closed by boy–boy are longer than those closed by girl–girl. Birth spacing tends to be longer among upper-income and Craftsman sub-caste mothers. The presence of a ‘grandmother’ in the household shortens spacing. These findings are compatible with an evolutionary-based reproductive decision-making process.

This study explores the relationship between poverty and vulnerability to HIV infection in Ethiopia using primary (quantitative and qualitative) and secondary data from two sub-cities of Addis Ababa. The data show that sexual experience is influenced by diverse factors such as age, gender, economic status and education level. Household economic status and migration explain the nature of sexual experience and level of vulnerability to HIV infection. Poor uneducated women in poor neighbourhoods are more likely to engage in risky sexual encounters despite awareness about the risk of HIV infection as they operate in an environment that provides the ‘path of least resistance’ (Lindegger & Wood, 1995, p. 7). This article argues that poverty provides a situation where early sexual initiation, ‘transactional sex’ and an inability to negotiate for safer sex are associated with low income, lack of education and increased vulnerability to HIV infection. This vulnerability is simultaneously contested and accepted as a commitment to even sacrifice one's life for the sake of one's loved ones. As a modest contribution to the ‘structural violence’ approach, which emphasizes social inequalities based on gender, class, ethnicity and race and inequalities in terms of exposure to risk and access to health care (Massé, 2007), this article challenges the ‘African promiscuity’ discourse, which ‘does not permit policymakers to think beyond sex’ (Stillwaggon, 2006, p. 156), and encourages researchers and policymakers to ask the right questions to understand the complexity of HIV/AIDS and seek solutions to the pandemic.

This study analyses factors associated with the incidence of sterilization regret in the four south Indian states of Andhra Pradesh, Karnataka, Kerala and Tamil Nadu. Using data from the National Family Health Surveys, in all four states the incidence of regret was found to be less than 10% and the factors significantly associated with it were child loss experience and quality of services. Hence, there is a need to improve the quality of services, both in terms of counselling and service provision, and women need to be counselled about the permanent nature of sterilization in order to avoid future regret.

This article, based on retrospective survey data, examines the fertility performance of third-order parity women in Korea, following births of specified sexes of children already born. Although the study indicates that additional births and spacing of children after the third birth are strongly influenced by the sex composition of the first three children, this influence becomes apparent only after the introduction of a vigorous national family planning programme. It seems that both the number of sons and the sequence of sons and daughters play an important role in determining subsequent fertility. More than twice as great a proportion of third-parity women proceeded to the fourth parity if the women had three daughters rather than three sons.

Several studies report that women exposed to intimate partner violence (IPV) are less likely to use contraception, but the evidence that violence consistently constrains contraceptive use is inconclusive. One plausible explanation for this ambiguity is that the effects of violence on contraceptive use depend on whether couples are likely to have conflicting attitudes to it. In particular, although some men may engage in violence to prevent their partners from using contraception, they are only likely to do so if they have reason to oppose its use. Using a longitudinal follow-up to the Indian National Family Health Survey (NFHS-2), conducted among a sample of rural, married women of childbearing age, this study investigated whether the relationship between IPV and contraceptive use is contingent on whether women’s contraceptive intentions contradict men’s fertility preferences. Results indicate that women experiencing IPV are less likely to undergo sterilization, but only if they intended to use contraception and their partners wanted more children (Average Marginal Effect (AME)=−0.06; CI=−0.10, −0.01). Violence had no effect on sterilization among women who did not plan to use contraception (AME=−0.02; CI=−0.06, 0.03) or whose spouses did not want more children (AME=−0.01; CI=−0.9, 0.06). These results imply that violence enables some men to resolve disagreements over the use of contraception by imposing their fertility preferences on their partners. They also indicate that unmet need for contraception could be an intended consequence of violence.

Data from interviews with 131 women who had a live birth and 109 who had a stillbirth about their experience of childbearing have been compared with information obtained from doctors.

For the live births medical information was more often obtained for those in teaching than in non-teaching hospitals. It is estimated that 84% of miscarriages, abortions and stillbirths were reported by women whereas 62% were recorded accessibly in the notes. Nerves and depression during pregnancy were reported much more frequently by the women than by the doctors.

Correlations over gestation were around 0·8. Concordance was greater over induction than over acceleration; it was high in relation to epidural anaesthesia and less good for other types of anaesthetic. Among the live births, reports of the baby's weight fell in the same half-pound group in 82% of instances and within adjoining groups for a further 12%. There appeared to be a tendency for mothers to report a slightly higher weight. In general agreement was less good for the stillbirths than for the live births.

Probable reasons for the discrepancies are discussed. Problems of precise definition, inaccuracies and inadequacies in the medical records and limitations of women's knowledge appeared to be the main sources of disparities.

313 pairs of brothers and 296 pairs of sisters were compared by one or both parents in a postal questionnaire. For 495 pairs independent questionnaires were completed by father and mother, and agreement between parents was good.

There was a marked tendency for the first-born child to be rated as less fond of cuddling; he was also easier to train, worked harder at school, set himself higher standards, and was more serious, methodical, law-abiding, tidy and less impulsive; he learned to talk and read at a younger age and was rated as having more natural ability at schoolwork. These three groups of first-born attributes were relatively independent; for instance, even the first-born with less ability at schoolwork were given more responsibility at school. In the 152 pairs which did not contain a first-born, little birth order effect could be found, whether or not the elder was the eldest of his or her sex.

There was a slight tendency for the broader, fatter and more muscular child to be less nervous and highly strung, but otherwise differences in physique were not related to differences in behaviour. Abilities at schoolwork, games, music and painting were not related. Tendency to anxiety and depression was related to lack of sociability; to stubbornness and lack of practicality; to angryness but not the bottling up of anger; to level of aspiration but not the possession of abilities or good looks. These relations of anxiety and depression in a normal sample closely parallel the items which distinguish psychiatric patients from their healthy siblings.
