This qualitative study explores the experiences of older adults participating in a creative visual arts program at a residential care facility in Victoria, British Columbia. A narrative inquiry approach was used to conduct face-to-face interviews with 10 residents and three program staff in addition to the systematic observations of program activities and an arts exhibit. The findings reveal the program fostered a sense of community among participants and enhanced their sense of self-worth as artists. A public art exhibition at a community centre underlined the value of residents’ artwork and gave meaning and purpose to their involvement in the program. Findings show the importance of arts programs in fostering creativity in later life and illustrate how people living in institutions can experience multiple dimensions of the self through artistic forms of expression. This study highlights the need to increase access to arts programs for individuals living in residential care.

Intersectionality analysis is the study of overlapping or intersecting social identities. Intersecting social identities may have an impact on the perception of burden by family caregivers of older persons with multiple chronic conditions (MCC). The purpose of this study was to explore the interaction of social factors on the burden of caring for older adults with MCC. A total of 194 caregivers of older adults with MCC were recruited from Alberta and Ontario. Survey data were collected at two time points, six months apart. Additive and multiplicative models were analysed using a generalised linear model to determine the level of caregiver burden. Medium-high social interference (impact on social life) was associated with higher burden when adjusted for age, gender, education, and employment status. The overall results of the five-way interaction suggest that males in general had lower burden scores than females. Irrespective of their education and employment status, females had generally higher burden scores. These results add to the current body of literature, suggesting areas for further research to fill knowledge gaps, and promoting ideas for evidence-guided public health interventions that focus on caregivers.

Case-Finding for Complex Chronic Conditions in Seniors 75+ (C5-75) is a systematic approach to identify frailty using gait speed and hand-grip strength and to screen for co-morbid conditions. We identified the C5-75 features offering the highest yield for identifying frailty and to streamline the screening program. Analyses included 1,948 C5-75 assessments completed from 2013 to 2018. Age 85 or older, less than regular physical activity, and more than two falls in the previous six months had the strongest associations with frailty. Exempting patients under 85 who reported regular physical activity and less than two falls excluded 39.1 per cent of the cohort while maintaining a sensitivity of 95.2 per cent and a negative predictive value of 99.4 per cent for frailty. These findings provide insight into optimizing screening for frailty, making it more feasible to implement and to identify co-existing conditions that may contribute to or be affected by frailty.

A community-based program is increasingly recognized as promoting health and active social participation in one’s life, yet information is lacking about the use and impact of such programs among immigrant visible minority seniors. This mixed-method research evaluated the impact of a cultural community program for Korean immigrant seniors by examining participants’ health-related quality of life (HR-QOL) benefits and overall well-being. In this study, 79 participants completed the SF-36v2 questionnaire twice to assess the impact of Canada Enoch Senior’s College (CESC) program on their HR-QOL and well-being. Statistically significant improvement in physical and mental health domains was observed: bodily pain and role limitations due to emotional problems. Qualitative data from participants’ interviews supported the survey findings with positive contributions in health and social arenas of seniors’ lives. These results suggest that the CESC program contributes to quality of life and well-being of Korean senior participants and supports similar community-based cultural programs.

The accelerated aging of the Canadian population is a recognized fact and leads to an increasing number of seniors with cognitive impairments (Curateur public du Québec, 2010a). This has a definite impact on health professionals who have to assess their competency to live independently and manage their finances. This decision, which has important consequences for the person, must be based on an objective and rigorous assessment. The purpose of this paper is to analyse the available tools, both in the scientific literature and in clinical settings, to better document the various components to assess seniors’ competency to live independently and manage their finances. The goal is to help practitioners who work with older people with cognitive impairments to accurately assess their ability to manage themselves and their property. A review of the relevant literature and training available, as well as three group consultations, showed that there is no consensus about the tools used to assess the capacity to take care of oneself and one’s property. Additional studies are thus needed to fill the gap in knowledge about specific tools used to assess competency.

Older adults are rarely consulted on health care restructuring. To address this gap, our study explored older adults’ views on “repositioning”, a restructuring initiative to support independent living for older adults with complex chronic disease (CCD). We collected and analysed data from 83 older adults living in one small city and nine rural small towns where “repositioning” of primary and community care was occurring. Average participant age was 75 years; 56 (67%) were women, 44 (53%) had CCD, and 20 (24%) identified as Indigenous or South Asian. The four themes were: unfamiliarity with repositioning; optimism versus skepticism; improving primary and community care (through better home care, improved transportation, and more doctors); and, playing an active role to effect change. For repositioning to be successful, diverse service users must be fully included; rural-dwelling older adults’ priorities for primary and community care need to be addressed, rather than using a “cookie-cutter” approach.

Existing cognitive health literature focuses on the perspectives of older adults with dementia. However, little is known about the ways in which healthy older adults without dementia understand their cognitive health. In rural communities, early dementia diagnosis may be impeded by numerous factors including transportation challenges, cultural obstacles, and inadequate access to health and support services.

Based on participant observation and two waves of 42 semi-structured interviews, this study examined healthy, rural older adults’ perceptions of cognitive health. By providing an innovative theoretical foundation informed by local perspectives and culture, findings reveal a complex and multidimensional view of cognitive health. Rural older adults described four key areas of cognitive health ranging from independence to social interaction. As policy makers, community leaders, and researchers work to address the cognitive health needs of the rural aging demographic, it is essential that they listen to the perspectives of rural older adults.

The 1982 World Assembly on Aging was convened to launch an international plan of action aimed at guaranteeing economic and social security to older persons, as well as opportunities for them to contribute to national development. The Plan of Action was to be considered as an integral component of major international development strategies and programs which have been formulated in response to important world problems and needs. This paper examines the background documents on aging prepared by the United Nations for world wide regional meetings held before the Assembly. It argues that the issues raised were restricted to a “social problems” perspective on aging which is congruent with the modernization theory of development. The economic and political dependency of Third World countries was ignored. As a result, the discussion and recommendations emerging from the regional meetings, in both developed and underdeveloped areas, focused only on policies and programs similar to those in industrialized countries. This paper concludes that such programs are irrelevant to the lives of most people as they age.

The purpose of this exploratory study was to develop, based on the ethnographic method, a research approach aimed at understanding the experience of widowhood as well as the coping strategies used by elderly men and women facing this experience. Ethnographic interviews were conducted at informants' homes, using a semi-structured format developed from a conceptual framework based on a stress and coping model. Interviews were tape-recorded and transcribed verbatim. Data were coded by two independent raters according to the study's conceptual framework and categories generated from the data. Results illustrate that personal factors such as biographical history, centrality of the spouse in personal life and self-efficacy influence the meanings of widowhood and the use of coping strategies; also, contextual factors, namely normative standards and cultural values, seem to have a differential influence on the meanings of widowhood and the coping strategies of widows and widowers. As a coping strategy, seeking social support is related to qualitative characteristics of social resources, more specifically reciprocity and reliability. The research approach developed in this project provides a means to better understand the contextual factors which might explain the great variability of mental health responses of elderly widows and widowers as well as elderly's responses to other stressors in their daily life.

One of the most influential applications of the concepts, methods and measures of social network analysis to the study of the social support transactions of older adults is the convoy model of social support. We draw on recent debates in network methodology to provide an assessment of the convoy model that explores the role of weak ties in the support networks of older adults. The social networks generated by the target diagram which operationalizes the convoy model display the structural and functional characteristics set out in its theoretical arguments. But not all of the ties constituting these networks are conduits of social support and, more importantly, these social networks do not include all supporting and supported others. The target diagram identifies core support networks; therefore, support flowing through weak ties is missed when it is used to set the boundaries of support networks. Expanding our picture of the support networks of older adults to take systematic account of weak ties and the emotional aid, instrumental assistance, and companionship that flow through them will enhance the effectiveness of support interventions that target older adults.

A recent trend in health promotion is to involve seniors in planning programs designed to meet their needs. However, many seniors have been socialized to be passive recipients of health care services and may initially require confidence-building and skill development to enable them to take an active role in health promotion.

This paper outlines the field development of a self-advocacy program. The central purpose of the program is to assist seniors to speak up for what they need to maintain control of their health and their lives. Results support the effectiveness of the program model as a framework for (1) helping seniors take increased initiative and control (2) developing a cohesive support network and (3) providing input from seniors into program planning. Recommendations are made for future research and program development.

Octogenarians living in the community are the fastest-growing demographic in Canada. Simultaneously, they have the highest prevalence of falls and nine times greater risk of injury due to a fall. To understand how to improve the safety of octogenarians’ aging-in-place, a systems approach is essential. Understanding how societal factors interact and affect the older adult can help care custodians identify and remove safety deficiencies that bring about falls. The purpose of this study was to identify system-wide factors contributing to falls in community-dwelling octogenarians. Eight falls were investigated using the systemic falls investigative method. Participants ranged in age from 83–90 years. Across-case analyses identified 247 contributing factors, grouped within four distinct themes: (a) everyday living has become risky; (b) supervision limitations; (c) health care system disconnects; and (d) poor fall risk identification and follow-up. This qualitative study provides systemic insights into how and why falls occur in community-dwelling octogenarians.

The coronavirus (COVID-19) pandemic and mandated physical distancing requirements significantly impacted volunteer programs for older persons with many long-standing programs either ceasing altogether or pivoting to connecting through virtual technologies. In this study, we collected qualitative interview data from 23 clients and 33 volunteers to investigate their experiences during the COVID-19 pandemic and the effects on the volunteer–client relationship. Three themes were identified: pandemic emotions, negotiating social interactions, and growing through the COVID-19 pandemic. These findings provide important insights into the experiences of hospice organizations and their volunteers and clients during the COVID 19 pandemic, further highlighting the importance of acknowledging both older persons’ vulnerability and their resilience, of building in compassionate community approaches to care, and of finding innovative ways to foster volunteer–client relationships during times when physical visiting is not possible.

Informal caregivers often experience high stress levels with little support, especially in rural settings. With a mixed-methods approach, this research explored experiences of rural informal caregivers, including how social identification as a caregiver, social interactions, and formal and informal coping support related to perceived stress. Major focus group themes (n = 8) included lacking available services, balancing challenges, unmet practical needs, and strong community identity. Survey data (n = 22) revealed that perceived coping support (e.g., having someone to turn to), social interactions, and caregiver identity (e.g., perceiving the role as important to one’s self-concept) were associated with lower life upset stress, but only caregiver identity was associated with managing the personal distress and negative feelings associated with caregiving stress. Results suggest that, although available rural services may fall short, other options might alleviate caregiver stress, including facilitating access to coping support, encouraging social interactions, and enhancing caregiver social identity.

COVID-19 has disproportionally impacted older adults, and has highlighted many issues, including extreme deficiencies in Canadian long-term care homes and gaps in home and community care services for older adults. In recent years, there has been a push towards better patient and family engagement in health system research, and with the onset of the pandemic, engaging older adults in research and policy planning is more important than ever. In this article, we describe the Seniors Helping as Research Partners (SHARP) approach to engagement with older adults as an example of how partnerships that engage older adults in the development of research aims and processes can help to ensure that future research meets the needs of older adults. SHARP members highlighted a number of areas for future COVID-19 research such as improvements to long-term care, enhancing access to home and community care, and a focus on aging and social isolation.

Interdisciplinary research using qualitative methods was employed to investigate the barriers to and facilitators of cultural participation for First Nations Elders who use wheeled mobility and live on reserve in Ontario, Canada. Themes were extracted into three broad categories: (1) impact of Elder participation in cultural activities, (2) usefulness of wheeled mobility devices, and (3) barriers to participating in cultural events. Barriers included lack of transportation, inability to access the outdoors safely and independently, challenging reserve terrain, stigma of being “disabled”, and feeling like a burden. Findings suggest that wheeled mobility can increase the active participation of Elders in cultural events, but demonstrate a need to facilitate the use of wheeled mobility on reserves in Ontario, Canada.

The maximum lifespan of different animal species is genetically determined. Many biochemical and physiological systems which influence longevity have apparently evolved to regulate growth and development in a way which maximizes fitness given the ecological niche and constraints on the species. The diversity of individual genetic effects on aging makes it unlikely that either extrinsic factors such as nutrition and medicine or genetic intervention will have dramatic effects on the maximum lifespan of a species, in spite of significant qualitative effects on individuals. However, understanding the fundamental genetic determinants of senescence may be of particular importance to the treatment and or prevention of age associated problems involving tissue degeneration and/or cancer. Considerable investment in basic biological research on both aging and developmental processes is needed before this understanding can be achieved.
