Using a unique set of birth registration data from the Demographic Surveillance System of the International Centre for Diarrhoeal Disease Research, Bangladesh, for the period 1974–77, and socioeconomic information collected in the 1974 census, fertility was studied in relation to occupation, size of dwelling, number of cows and number of boats owned. The total fertility rate was found to vary between 6 and 6·5 except in the famine year of 1975. There was no consistent relationship between fertility and education of women. Fertility differentials by occupation showed that the household heads who were farm labourers had relatively lower fertility compared to other occupational groups, except for the year 1977 where the families of service holders were found to have relatively lower fertility. There was a consistent direct relationship between the dwelling size and fertility for each of the years.

Variations in sex ratio with maternal age, paternal age, parity and time have been examined in data on all legitimate births (live and stillbirths) in England and Wales, 1968–77. There were significant linear declines in sex ratio with both increasing parity and increasing paternal age, and a curvilinear relationship with maternal age. These effects were largely independent. Before 1955 it seems likely that sex ratio declined with increase in maternal age. But it seems that between 1955 and 1968 there was an unexplained decline in the sex ratio of births to women aged 20–24 and an increase in that of births to women aged 30–34. There was no significant secular trend in sex ratio during the period 1968–77.

In 1984 and 1985, a survey was conducted of 7696 women aged 15–44 living in Italy, France, Great Britain, Spain and the Federal Republic of Germany. The aim of the study was to examine the use of contraceptive methods, the differences in contraceptive use, knowledge of fertility, communication about contraception, motives for choice and the perceptions held by women regarding contraceptive methods, particularly oral contraception. The results show important differences between the countries studied.

This study compares the effects of two distinct forms of human capital – income and education – on marital status and childlessness separately by sex in six different countries. Nearly 10 million individual records on individuals aged 16 to 50 were used from censuses from Brazil, Mexico, Panama, South Africa, USA and Venezuela dating from 2000 or later, to analyse the relationship between education, income and marital status and childlessness in men and women. Regarding income, the findings for both outcome variables are strongly consistent across all six countries. Highest-income males and lower-income females have the highest proportion of ever-married and the lowest proportion of childlessness (using a proxy for childlessness: own children in the household or not). There is no corresponding consistency of findings as regards education either between the sexes or among the countries. To conclude, a lower percentage of low-income men are selected by females, because for women male status and resources provided by men are important criteria in mate selection. Therefore a higher proportion of low-income men remain unmarried and childless. Thus selection seems to play a role in modern societies.

The purpose of this study was to investigate whether the well-established relationship between parity and birth weight is affected by the sex composition of siblings, especially for male newborns. Subjects were 856 male and 862 female newborns who weighed at least 2500 g at birth, who were born after 37 completed weeks of gestation, who obtained an Apgar score of 7 or higher, who had the same biological parents as all other children in the sibship, and who lived in the same household. Information on birth weight was collected from hospital records. Results showed that male newborns with older brothers weighed less than male newborns with older sisters. In contrast, the weight of female newborns with older brothers did not differ from the weight of female newborns with older sisters. One explanation of these results is that maternal immunoreactivity to some male-specific feature of the fetus affects prenatal development and consequently reduces birth weight in males. The relation between older brothers and birth weight may have theoretical significance for behavioural variables.

The effects of socioeconomic factors on secular trends in suicide rates in Japan for the periods 1953–72 and 1973–86 were investigated using twelve socioeconomic indicators. Multiple regression analysis showed that the socioeconomic indicators affecting suicide rates were not identical in the two periods. The rates in both sexes in 1953–72 were closely related to unemployment rate and the labour force but between 1973 and 1986, divorce rate and the proportion in tertiary industry were most influential. The changes reflect the socioeconomic changes in industrial structure in Japan in transition from an industrial to a service economy.

Variation in the duration and pattern of breast-feeding contributes significantly to inter-population differences in fertility. In this paper, measures of suckling frequency and intensity are used to compare the effects of breastfeeding practices on the duration of lactational amenorrhoea, and on the length of the birth interval in three prospective studies undertaken during the 1980s, among Quechua Indians of Peru, Turkana nomads of Kenya, and Gainj of Papua New Guinea.

In all three societies, lactation is prolonged well into the second year postpartum, and frequent, on-demand breast-feeding is the norm. However, the duration of lactational amenorrhoea and the length of birth intervals vary considerably. Breast-feeding patterns among Gainj and Turkana are similar, but Turkana women resume menses some 3 months earlier than do the Gainj. The average birth interval among the Gainj exceeds that of nomadic Turkana by over 15 months. Suckling activity decreases significantly with increasing age of nurslings among both Gainj and Quechua, but not among Turkana. Earlier resumption of menses among Turkana women may be linked to the unpredictable demands of the pastoral system, which increase day-to-day variation in the number of periods of on-demand breast-feeding, although not in suckling patterns. This effect is independent of the age of infants.

The short birth intervals of Turkana women, relative to those of the Gainj, may be related to early supplementation of Turkana nurslings with butterfat and animals' milk, which reduces energetic demands on lactating women at risk of negative energy balance.

African men play important roles in the decisions about family life, including fertility and family planning. However, fertility and family planning research and programmes have ignored their roles in the past, focusing only on women’s behaviours. Since the 1994 International Conference on Population and Development (ICPD), interest in men’s involvement in reproductive health has increased. Unfortunately, data on their knowledge and use of contraception are generally scanty. This paper examines knowledge and use of contraception among ever-married men in Nigeria. A total of 1451 ever-married men aged 18–55 were interviewed in Imo and Ondo States, Nigeria. The findings reveal that men’s level of contraceptive knowledge is high in the study areas. About 90% knew at least one method of family planning. Furthermore, the level of contraceptive use among married men is such that men could participate in family planning activities if there were adequate programmes to involve them. Men in the sample areas were found not only to support their spouses’ use of contraceptives, but were actually using condoms to delay or prevent pregnancy. Age, education, place of residence, number of living children and being counselled for family planning were identified as key factors determining contraceptive knowledge and use among married men in the study areas. To ensure increased participation of men in family planning, programmes must be designed to educate them on the need for family size limitation and involve them in service delivery, even if only to their male counterparts.

The Journal of Biosocial Science regularly publishes papers addressing the social and cultural aspects of disease, sickness and well-being. Most of these papers attempt to understand the prevalence and distribution of disease and sickness within and between populations as well as local responses to biomedical interventions and public health policy more generally. They fall broadly within the remit of human ecology; and they embrace a ‘factorial’ model of disease in which social and cultural factors are deemed to be just one of a number of factors to be considered alongside a range of other factors. These include biological features of the infecting organism; nutritional factors; environmental factors; psychological factors; and genetic factors influencing susceptibility to disease at an individual and population level.

This paper critically analyses from a political sociology standpoint the international conceptualization of war-affected populations as traumatized and in need of therapeutic interventions. It argues for the importance of looking beyond the epidemiological literature to understand trauma responses globally. The paper explores how the imperative for international psychosocial programmes lies in developments within donor countries and debates in their humanitarian sectors over the efficacy of traditional aid responses. The aim of the paper is threefold. First, it discusses the emotional norms of donor states, highlighting the psychologizing of social issues and the cultural expectations of individual vulnerability. Second it examines the demoralization of humanitarianism in the 1990s and how this facilitated the rise of international psychosocial work and the psychologizing of war. Third, it draws attention to the limitations of a mental health model in Croatia, a country which has been receptive to international psychosocial programmes. Finally it concludes that the prevalent trauma approaches may inhibit recovery and argues for the need to re-moralize resilience.

Consanguineous marriages in two population samples, one rural and one urban, from Swat (Pakistan) were studied. The frequency of consanguineous marriages was found to be 37·13% and 31·11%, and mean inbreeding coefficients were calculated as 0·0168 and 0·0162, for the rural and urban populations respectively. The most frequent type of marriage was between first cousins, in both samples. Among first cousin marriages, those with father's brother's daughter were predominant. Mean inbreeding coefficient was higher for higher socioeconomic groups in both samples. Differences by ethnic and educational groups were also found. Contrary to previous studies, a significant increase in the incidence of consanguineous marriages over the years has been observed. The incidence of premature mortality was significantly higher only in the offspring of first cousin marriages. Significantly higher incidence of morbidity in the offspring of consanguineous marriages was also observed.

This is an exploratory study to examine the social and cultural determinants of the teaching of HIV/AIDS sex education among secondary school teachers in Eastern Nigeria. The research analyses how teachers perceive passing their knowledge of HIV/AIDS prevention measures to their students in the context of their cultural and social norms, which restrict open discussion of sex. This is a qualitative study based on in-depth interviews with 60 teachers drawn from secondary school teachers in Eastern Nigeria, supplemented with five focus group discussions, and content analysis of teachers’ lesson preparatory notes. The findings show a high level knowledge of HIV/AIDS preventive measures among teachers. However, teachers are not passing on this knowledge because of cultural and social inhibitions. In addition, teachers have not been receiving adequate training and motivation on information, education and communication for HIV/AIDS sex education. The situation calls for serious policy intervention.

Although gains in generational intelligence test scores have been widely demonstrated around the world, researchers still do not know what has caused them. The cognitive stimulation and nutritional hypotheses summarize the several diverse potential causes that have been considered. This article analyses data for a sample of 499 children tested in 1930 and one equivalent sample of 710 children tested 72 years later, the largest gap ever considered. Both samples comprised children aged between 7 and 11 who were assessed by the Draw-a-Man test in the city of Belo Horizonte, Brazil. Further, one additional sample of 132 children was assessed in 2004 in a rural area very similar in several diverse factors to the 1930 urban sample. The results are consistent with both the cognitive stimulation and the nutritional hypotheses.

The timing of transitions to sexual activity, marriage and childbearing in sub-Saharan Africa is undergoing profound changes. This study investigates the determinants of adolescent transitions in South Nyanza, a socioeconomically deprived setting in Kenya where adolescent reproductive health is a particular concern. The analysis is based on Cox regression of timing of first sexual intercourse, first marriage and first pregnancy, using data from a survey of 1247 females aged 12–19 years. The results show that higher household socioeconomic status and educational attainment are associated with delayed onset of all three transition events. Furthermore, mother’s higher educational attainment is protective for initiation of sexual intercourse while rural residence is protective for pregnancy experience. Other protective factors include communication with parents or with fellow girlfriends. However, discussing sexual matters with boyfriends, high internal locus of control, and gender bias are associated with early onset of the three transition events.

A study on perceptions, attitudes and treatment-seeking practices related to schistosomiasis was conducted among the Wasukuma in the rural Magu district of Tanzania at the shore of Lake Victoria where Schistosoma haematobium and mansoni infections are endemic. The study applied in-depth interviews, focus group discussions and a questionnaire survey among adults and primary school children. The perceived symptoms and causes were incongruous with the biomedical perspective and a number of respondents found schistosomiasis to be a shameful disease. Lack of diagnostic and curative services at the government health care facilities was common, but there was a willingness from the biomedical health care services to collaborate with the traditional healers. Recommendations to the District Health Management Team were: that collaboration between biomedical and traditional health care providers should be strengthened and that the government facilities’ diagnostic and curative capacity with regard to schistosomiasis should be upgraded. Culturally compatible health education programmes should be developed in collaboration with the local community.

The latter part of the twentieth century has seen an increased concern for the implications of war for civilian populations, and more attention has been given to psychosocial impacts of uprooting and displacement. ‘Loss of place’, acute and chronic trauma, family disruption and problems of family reunification have become issues of concern. The war in Bosnia was characterized by massive displacement, disruption and loss of life, relatives and property. Health and psychosocial well-being were affected in a number of ways. There was an overwhelming loss of perceived power and self-esteem. Over 25% of displaced people, for example, said they no longer felt they were able to play a useful role; even in non-displaced populations approximately 11% of those interviewed said that they had lost a sense of worth. Widespread depression and feelings of fatigue and listlessness were common and may have prevented people from taking steps to improve their situation. Almost a quarter of internally displaced people had a high startle capacity and said they were constantly nervous. Most adverse psychosocial responses increased with age and in a population that includes many elderly people this poses serious problems. The findings point to major challenges with respect to repatriation and reconstruction. They highlight the importance of family reunification and the facilitating of decision-making by affected people themselves. The findings also shed light on potential problems associated with over-dependence on external assistance and hence the need for people to be given the means of using their skills and knowledge to control their day-to-day lives.

Domestic violence and obesity are global public health problems. This study explores associations of domestic violence and obesity in 5015 ever-married, non-pregnant women aged 15–49 years who took part in the 2005 Egypt Demographic and Health Survey (EDHS). Women's mean body mass index (BMI) was 30.4 kg/m2, and 48% were obese. Thirty-seven per cent reported any prior psychological, physical or sexual domestic violence. Compared with their counterparts, the adjusted odds of being obese were marginally higher among women who reported any prior sexual (aOR=1.31), physical or sexual (aOR=1.18), or psychological, physical or sexual (aOR=1.17) domestic violence. Women who experienced severe or repeated domestic violence – as measured by reported exposure to three or more acts of physical (aOR=1.25), psychological or physical (aOR=1.18), physical or sexual (aOR=1.36), and psychological, physical or sexual (aOR=1.26) domestic violence – had higher adjusted odds of being obese. Marginally significant adjusted dose–response relationships remained between obesity and the number of specific acts of: (1) physical or sexual violence and (2) psychological, physical or sexual domestic violence. Obesity among women in poorer settings like Egypt may partly have its roots in gender subordination, as manifested in women's exposure to multiple acts of domestic violence.

Abortion is illegal in Uganda except to save the life of the woman. Nevertheless, the practice is quite common: about 300,000 induced abortions occur annually among Ugandan women aged 15–49 (Singh et al., 2005) and a large proportion of these women require treatment for post-abortion complications. In the male-dominant culture of Uganda, where men control most of the financial resources, men play a critical part in determining whether women receive a safe abortion, or appropriate treatment if they experience abortion complications. This study examines men's roles in determining women's access to a safer abortion and post-abortion care. It draws on in-depth interviews carried out in 2003 with 61 women aged 18–60 and 21 men aged 20–50 from Kampala and Mbarara, Uganda. Respondents' descriptions of men's involvement in women's abortion care agreed that men's stated attitudes about abortion often prevented women from involving them in either the abortion or post-abortion care. Most men believe that if a woman is having an abortion, it must be because she is pregnant with another man's child, although this does not correspond with women's reasons for having an abortion – a critical disjuncture revealed by the data between men's perceptions of, and women's realities regarding, reasons for seeking an abortion. If the woman does experience post-abortion complications, the prevailing attitude among men in the sample was that they cannot support a woman in such a situation seeking care because if it had been his child, she would not have had a covert abortion. Since money is critical to accessing appropriate care, without men's support, women seeking an abortion may not be able to access safer abortion options and if they experience complications, they may delay care-seeking or may not obtain care at all. Barriers to involving men in abortion decision-making endanger women's health and possibly their lives.

This article describes the long-term trends in marital status mortality differences in the Netherlands using a unique dataset relating to the period 1850–1970. Poisson regression analysis was applied to calculate relative mortality risks by marital status. For two periods, cause-of-death by marital status could be used. Clear differences in mortality by marital status were observed, with strongly increasing advantages for married men and women and a relative increase in the mortality of widowed compared with non-married people. Excess mortality among single and formerly married men and women was visible in many cause-of-death categories, and this became more widespread during the last decades of the nineteenth century. Hypotheses are formulated that might explain why married men and women underwent a stronger decrease in mortality up until the end of World War II.
