This article offers reflection on the validity of relational data such as used in social network analysis. Ongoing research on the transformation of the support network of caregivers of persons with an Alzheimer-type disease provides the data to fuel the debate on the validity of participant report. More specifically, we sought to understand the factors that might influence the description of the support network by persons involved in caregiving. The issue warrants special attention, given that social relations – in their form and their content – constitute the raw material of network analysis. We propose that how persons describe their social network corresponds to a subjective process that rests, in part, on their representation of their cultural and social universe.

This study examined the impact of the 1998 Canadian ice storm on the physical and psychological health of older adults (age > 55 years) living with a chronic physical illness, namely osteoarthritis and/or osteoporosis. Although disasters are relatively rare, they are a useful means of examining the impact of a single stressor on a group of individuals. Specifically, we took advantage of a natural experiment to compare the responses of a group of 59 ice storm victims to those of 55 matched controls living outside the ice storm area. Data on disability, pain, self-reported health, helplessness, depression, and independence were assessed prior to the ice storm and approximately 17 months later. Older adults who reported greater helplessness and lost independence prior to the storm reported significantly greater ice storm stress and rumination and were more likely to report that the storm affected their condition. In addition, participants exposed to the ice storm reported significant changes in disability and pain nearly a year and a half later, compared to matched controls. These results suggest that older adults with chronic physical illness may be particularly vulnerable when faced with additional stressful events.

This study sought to develop frailty “identification rules” using population-based health administrative data that can be readily applied across jurisdictions for living and deceased persons. Three frailty identification rules were developed based on accepted definitions of frailty, markers of service utilization, and expert consultation, and were limited to variables within two common population-based administrative health databases: hospital discharge abstracts and physician claims data. These rules were used to identify persons with frailty from both decedent and living populations across five Canadian provinces. Participants included persons who had died and were aged 66 years or older at the time of death (British Columbia, Alberta, Ontario, Quebec, and Nova Scotia) and living persons 65 years or older (British Columbia, Alberta, Ontario, and Quebec). Descriptive statistics were computed for persons identified using each rule. The proportion of persons identified as frail ranged from 58.2-78.1 per cent (decedents) and 5.1-14.7 per cent (living persons).

This study compared the changes in some bio-psychosocial variables (functional independence, nutritional risk, pain, balance and walking, grip strength, general well-being, psychiatric profile, perception of social support, leisure satisfaction, and caregivers' feeling of burden) in four categories of clients during their program at a geriatric day hospital (GDH). The study also evaluated whether or not improvements, if any, were maintained 3 months after discharge. One-hundred-and-fifty-one people, categorized by primary reason for admission, were assessed at the GDH with reliable and valid tools, at admission and at discharge. Three months after discharge, they were reassessed with the same tools. Overall, two categories of clients, stroke / neurological diseases and musculoskeletal disorders / amputations, improved the most. For the gait disorders and falls group, only the functional independence score improved, but not at a clinically significant level. Finally, clients in the cognitive function disorders / psychopathologies group improved the most on their well-being scores and caregivers' burden decreased the most. All gains were maintained up to 3 months after discharge, except for leisure satisfaction. With the exception of clients who attended the GDH because of gait disorders and falls, the improvements and maintenance achieved in each category occurred in the domains where improvement had been hoped for, because of the particular disabilities in question and because of the nature of the GDH services offered.

Current scholarship in the sociology of age relations suffers from conceptual ambiguity and confusion in the area of age and generational relations. This paper recommends precise definition and use of the terms cohort, generation, age grade, age stratum, age group, and generation groups. Concrete or real definitional constructs are distinguished from nominal constructs and their advantages are described. The central thrust of the paper is a call for a return to the conceptualization of Mannheim about generations and a parallel conceptualization concerning age groups. The usefulness of this approach is discussed in terms of the political sociology of age relations.

This qualitative descriptive study presents the experience of an abrupt disruption of daily activities among community-dwelling older adults during the early months of the COVID-19 pandemic. Sixteen older adults (age: 71 ± 6.4) were interviewed in April–June, 2020. Five themes were identified: (1) Understanding and applying COVID-19 guidelines: guidelines were clear and participants adhered closely to them, motivated mainly by fear; ageing-specific guidelines are needed. (2) Daily life during lockdown: the abrupt occupational disruption was managed by transitioning to virtual activities, and/or performing more activities at home. New daily activities were generally more sedentary and less meaningful. (3) Social context: family assistance aroused mixed feelings, as it compromised independence; limited compliance at the community level created stress. (4) Mood and affect: mood often fluctuated, and participants employed various coping strategies. (5) Aging: participants became more aware of their age and were concerned about negative health implications of adherence to COVID-19 guidelines.

A mail-out survey method was used to assess the transportation needs of seniors living in relatively remote rural areas of Saskatchewan. In all 1675 questionnaires were returned for an 87% return rate. Results showed that the transportation-disadvantaged were very old widowed women living alone on a small income in a village or small town who were particularly likely to be lonely, in poor health, and physically frail. Two types of transportation needs were identified: the need to make local trips at least once a week and the need for transportation to a district centre at least once a month. Examples of volunteer, locally-run transportation systems that might meet these needs and be cost-effective are reviewed.

This research presents risk factors for abuse and neglect among older adults who accessed health and social service organizations in London, Ontario, 1991. The paper reviews four theoretical perspectives on elder abuse and neglect: the situational model, social exchange theory, symbolic interactionism and a feminist model. Risk factors for physical abuse, chronic verbal abuse, material abuse and neglect were identified for each theoretical model. Each set of risk factors was regressed on each type of abuse in order to determine the efficacy of the theoretical models. The data suggest that the risk factors associated with the situational and feminist model(s) provide the best fit in terms of physical abuse; the symbolic interaction model explains the most variance in chronic verbal abuse; both the symbolic interaction and feminist models explain about the same amount of variance in neglect; and none of the theoretical models adequately explains the variance in material abuse.

Cette étude a exploré la signification d’une relation positive client-infirmière pour les personnes âgées atteintes de maladies chroniques qui reçoivent les soins à domicile. Pour les besoins de cette étude phénoménologique, huit participants âgés de 65 à 86 ont été sélectionnés à partir du Sud-Ouest Centre communautaire d’accès aux soins [or retain as Southwest Community Care Access Centre (SW-CCAC)]? à London, Ontario. Des données narratives ont été recueillies par des entrevues en profondeur capturé sur cassette audio, tout en utilisant un guide d’entretien semi-structuré. L’analyse et l’interprétation des entrevues ont révélé que le sens d’une relation client-infirmière positive de ces personnes âgées englobait deux tendances de sens: avoir de confort, et étant relié au sein de cette relation. Ces deux modèles ont été contextualisés par être une personne âgée avec une maladie chronique et ont été socialement construits à travers l’expérience d’avoir créé un rapport positif. Bien que des recherches supplémentaires sont nécessaires, les connaissances acquises ajouter à ce que l’on sait déjà au sujet de la théorie et la pratique de la promotion de la santé relationnelle pour les personnes âgées vivant avec des maladies chroniques et recevant les soins à domicile.

This article discusses developments in theory and research on happiness two decades after publication of Psychological Well-Being in Later Life (Butterworths, 1991) by Albert Kozma, Michael Stones, and Kevin McNeil. Major empirical advances include new knowledge about contributions to happiness resulting from genetically related effects and personality. Personality traits have stronger relationships with happiness than was apparent 20 years ago and contribute to covariance between happiness and some of its predictors. Evolving emphases in research include the ways in which genetically related effects influence how people shape, and react to, their environment.

Bathrooms are a common location for falls among older adults. Bath grab bars can assist in promoting safe bath transfers. The aim of this study was to identify predictors of bathroom safety-device use among community-living seniors. A two-stage sampling strategy was used to select, first, a random sample of non-universal apartment buildings and a matched sample of universal buildings, from among non-profit apartment buildings in two Canadian regions; and second, a random sample of participants within each building. A total of 550 seniors participated in face-to-face interviews in their apartments. Participants within each building type presented with similar profiles. A logistic regression was used to identify predictors of grab-bar use among participants who had grab bars and entered the bathtub on a regular basis (n = 478). Significant predictors, in order of odds ratios, were bathing difficulties, ease of grab-bar use, living in buildings with policies supporting universal access to grab bars, having a history of falls, and reporting few psychosocial consequences of grab-bar use. Findings of this study emphasize the importance of promoting access as a key strategy for increasing use and have important implications for policy planning and falls-prevention initiatives.

Échantillonnage des ménages de l'Ontario de la section de la santé de l'Enquête nationale sur la santé de la population de 1994–1995 a été relié à la base de données du Ontario Home Care Administrative System et l'on a exploré la relation entre les réponses des personnes interrogées et l'utilisation des soins à domicile dans les années qui ont suivi l'enquête dans le but d'évaluer les variables prédictives des soins à domicile. La prévalence de l'utilisation des soins à domicile chez les sujets de l'enquète s'élevait à 4,3 pour cent. Les facteur indépendants associés à une augmentation probable des soins à domicile étaient un age de 75 et plus, une contrainte des activités quotidiennes (AVQ) et des activités instrumentales de la vie quotidienne (AIVQ), une détérioration de la santé et l'utilisation antérieure de soins à domicile. Il faudra approfondir les travaux pour mieux comprendre les facteurs associés à l'utilisation des soins à domicile si l'on souhaite mieux définir le but et les objectifs des soins à domicile et contribuer à l'élaboration de modèles de financement de ces soins.

An understanding of trends and determinants for the residential mobility of elderly Canadians is essential for public policy and planning. Study of the patterns, changes over time, and determinants of the mobility of older Canadians has become increasingly important as the population ages. Elderly residential mobility has decreased substantially since 1971, and almost one-half of this decrease is due to changes in population composition. Because the multivariate analysis described here does not account for most of the downward trends in residential mobility, however, further work is needed on speculative explanations discussed in this article.

Population aging is likely to lead to an increase in the number of people in need of assistance. It is well known that a large part of this assistance originates, and will continue to originate, from the network of relatives and friends. However, the effects of the provision of care on individuals’ employment trajectories when this care is combined with employment of varying intensity or with childcare responsibilities have rarely been examined. The present study used proportional hazards models with the General Social Survey, Cycles 20 and 21, to assess the impact of providing care to a partner, a parent or parent-in-law, another relative, or a non-relative on the risk of leaving employment. The analyses show that providing care to a parent or parent-in-law increases the probability of leaving employment only among women employed full-time and among men and women who have no children or only adult children.

Supportive housing, including retirement homes and assisted living, is increasingly touted as a suitable living option for Canadian older adults. This scoping review describes the nature and content of studies that explore underlying factors that motivate older adults to relocate to supportive housing. We conducted a search of PubMed, Cumulative Index of Nursing and Allied Health Literature (CINAHL), Web of Science, and PsycINFO, which identified 34 articles for review. Articles reviewed employed a variety of methods and guiding theoretical frameworks, of which the push and pull framework appeared to be most common. This review suggests that health and functional deficits are important reasons for relocation to supportive housing for older adults. Further longitudinal data are required to more comprehensively describe medical and social determinants for relocation and its consequences, in order to better describe this growing population and better align policies with the needs of older adults contemplating or undergoing relocation.

Rural-urban differences in the characteristics of 44 individuals entering the province-wide Adult Day Care program throughout Manitoba are examined. In addition, the rural-urban differences along various social and social psychological indicators after participants have been in the program are explored. The results indicate that there are many similarities and relatively few differences between the urban and the rural ADC participants. Any differences which do emerge portray the rural participants as more socially integrated and with higher levels of perceived health than the urban participants. Implications for assessment for entry into Adult Day Care are discussed.

Standard clinical diagnostic procedures are often inappropriate and frequently not feasible to apply in population-based studies, yet ascertaining accurate disease status is essential. We conducted a systematic review to identify algorithms, criteria, and tools used to ascertain 17 chronic diseases, and assessed the feasibility of developing algorithms for the CLSA. Of the 29,616 citations screened, 668 papers met all inclusion criteria. We determined that the information included in a disease algorithm will differ by condition type. The diagnosis of some symptomatic conditions, such as osteoarthritis and arthritis, will require substantiation by clinical criteria (e.g., x-rays, bone density measurement) while other conditions, such as depression, will rely solely on self-report. Asymptomatic conditions, such as hypertension, are more difficult to ascertain by self-report and will require additional physiologic measures (e.g., blood pressure) as well as laboratory measures (e.g., glucose). This pilot study identified the tools necessary to develop disease ascertainment algorithms.

The move to a long-term care facility can be particularly traumatic for new residents. Staff can make this transition easier in a number of ways. However, the staff’s perceptions of the transition process and residents’ experiences will play a significant part in determining the type of support that is given residents during the transition. The purpose of this research was to examine the staff’s perceptions of a person’s coming to live in a long-term care environment. Using in-depth interviews with staff from one long-term care facility, three main themes emerged that encompassed descriptions of residents’ lives. Essentially, the staff described how residents learned to live a life involving various factors in three main categories—life around losses, life around the institution, and life around the body.

This study utilized 1985 United States National Ambulatory Medical Care Survey data to explore the relationship between the drug prescribing phenomenon, measured by inappropriate prescribing and by prescription volume, and patient age. Inappropriate prescribing was relabelled potentially undesirable prescribing because limited medico-social patient information restricting circumstantial knowledge could result in an oversimplified and unduly critical judgment of prescribing. Categories of potentially undesirable prescribing were developed: 1) more than one drug in the same class; 2) drug classes not to be combined; 3) drugs serving sub-optimal treatment; and 4) drugs and diagnoses not to be combined. Of 67 different generic drugs representing 13 drug classes studied, 242 products were prescribed to the sample of 33,574 patients age 45+ years. Potentially undesirable prescribing was found to be a common problem regardless of type of medical practice and the sex of the patient: it is related to the age of the patient. The adverse effect of visiting a general practitioner is greater for older patients; and the adverse effect of attending a practice in a non-metropolitan area is larger for the older patients.
