The purpose of this study was to investigate the phenomenon of spousal caregiving following the institutionalization of elderly husbands. Theoretically, the study was informed by the interpretive perspective in sociology and the concept of career. Methodologically, the study employed a longitudinal and prospective design and combined quantitative and qualitative approaches. The data were drawn from a larger study designed to explore the transition to quasi-widowhood. This article reports on one aspect of spousal caregiving following the admission of husbands to a long term care setting, i.e. the performance of tasks. In this study, wives carried out a substantial number and wide variety of tasks related to personal, instrumental, relational and recreational care. They viewed visiting as their most important task but also highly valued providing companionship, love and support, doing little things for their husbands and monitoring their health and well-being. Typically, wives carried out tasks they viewed as falling within their domain of responsibility. Task performance occurred within a context of meaning that served to explain, in part, their continued involvement in caregiving following the admission of husbands to a long term care setting. Implications of this study point to the need for collaboration in caregiving between wives and staff, to support and enhance the quality of care provided by elderly wives who are coping with the institutionalization of their husbands.

We examined ageist beliefs, attitudes, and knowledge towards sexuality in later life among 148 nursing students and 137 social work students in their first year of studies. The students completed the Fraboni Scale of Ageism, the Attitudes and Knowledge towards Older Adults’ Sexuality Scale, and questionnaires regarding contact with older adults, prior sexual education, and socio-demographics. The results indicated that nursing students have more conservative attitudes towards older adults’ sexuality. Ageist beliefs correlated positively with more conservative attitudes towards older adults’ sexuality. These conservative attitudes ’were negatively correlated with knowledge about sexuality in later life. Older students have more knowledge about sexuality in later life and more permissive attitudes towards older adults’ sexuality. Religious students have less knowledge, and more conservative attitudes, towards older adults’ sexuality. Previous sexual education was not associated with the students’ attitudes and knowledge towards older adults’ sexuality. Implications for designing continuing education programs are discussed.

The aim of the present investigation was to determine whether differences in the strength of original information influence adult age differences in susceptibility to misinformation. One-half of the younger and older adults watched a slide sequence once (one-trial learning) that depicted a theft, whereas the remaining participants viewed the slide sequence repeatedly to ensure that all critical details were encoded (criterion learning). Three weeks later and immediately prior to final testing, participants were asked questions that contained misleading information. As expected, the degree of initial learning influenced age differences in misinformation reporting. That is, when event memory was poorer for older than younger adults (in the criterion learning condition), older adults were more susceptible to misinformation than younger adults. However, when memory of the event was poor (in the one-trial learning condition), the younger adults reported more misled details than the older adults, possibly because the younger adults had better memory for the misleading information. Therefore, strength of initial memory influences the extent and direction of adult suggestibility and helps explain the discrepancy found across studies in this area.

Given the rapidly expanding older adult population, finding health care approaches that support older adults to age in their choice of place, with an accompanying philosophical re-orientation of health services, is becoming more urgent. We studied the Home Care Home First – Quick Response Project to understand how clients over age 75 and their family caregivers perceived the enhanced community-based services delivered through Home First. Using interpretive description as the methodological design, we explored the experiences of eight older adults and 11 family caregivers; all older adults were enrolled in Home First due to a significant change in their health status. We identified four themes: growing older in chosen places with support, philosophy of care, processes of Home First, and the significance of Home First for clients. Overall, clients and family caregivers responded positively to the Home First services. Clients valued their independence and growing older in places they had specifically chosen.

This article addresses the question: What is the role of languaging – the shaping and organizing of higher mental processes through language – in emerging zones of proximal development (ZPDs) co-created by two adults? The two adults are a resident in a long-term care facility (Mike) and a researcher. A ZPD is an ongoing cognitive/affective activity in which learning and development occur as participants interact. This process is mediated by languaging. Through a microgenetic analysis of selected representative excerpts from 11 one-on-one sessions, we illustrate how the interactions between Mike and the researcher create a positive affective context which affords multiple opportunities for ZPDs to emerge. During the emergent ZPDs, we observe how languaging brings together the cognitive and affective components essential for the participant’s continued development. Over time, Mike reclaims lost expertise and takes on new complex cognitive challenges. Outcomes for Mike include both cognitive development and enhanced self-esteem.

Although the physical environment as a co-determinant of health could be approached in many ways, we chose to focus on an understudied area: how the immediate living environment can act as a persuasive force affecting physical activity level in older people, with physical activity in turn affecting health status. To explore this topic, the methods and findings of a literature search are described, the theoretical underpinnings of our thesis are presented, and an example is given of how a planned environment, which upon first glance seems supportive, may have unexpected and negative consequences on the activity level, and ultimately the health status, of the residents. Theory related to changes with aging in homeostatic capacity and reserve capacity of organ systems as well as Lawton's environmental press-competence model are applied to environmental characteristics (e.g., staircases) of continuing care retirement communities. We argue that physically challenging aspects of the environment, such as stairs, should be included in the design of living spaces for the elderly with the goal of encouraging greater daily physical activity and improved health status.

The aging of the population is expected to result in substantial increases in the costs of maintaining health care and pension programs, and that is a source of widespread concern. However, a proper assessment requires that attention be given to all categories of government expenditure, including education and others associated with younger age groups, and not just those associated with the older population. It requires also that privately provided goods and services be considered, since their costs must be charged against the same national income as publicly provided ones. Beyond that, it is important to recognize that population change affects not only the demand side of the economy, but also the supply side — the economy's productive capacity. An important conclusion is that while other influences will no doubt play a role, demographic effects by themselves are likely to cause government expenditure (all categories, all levels of government combined) to increase by no more than the rate of growth of the population, and by less than the rate of growth of the gross domestic product. Taking public and private costs together, and assigning appropriate weights to different age groups, the overall “dependency ratio” can be expected to remain at its current low level for another decade and a half or two decades, and then to rise as the baby boom generation retires in large numbers. However, the projected future ratio never reaches the levels of the 1950s and 1960s. Although the overall “burden” of population aging is manageable, major adjustments will be required in the coming decades, especially in the area of federal/provincial cost sharing. For the most part, though, the effects of population aging are predictable, slow, and some time off.

Previous research on multigenerational households in later life has generally ignored the distinction between multigenerational households located in the home of a parent and those located in the home of a child. This distinction has implications for dependency on the part of the older adult and also for social policy which is primarily concerned with the child-provided type of multigenerational household. The present analysis employs the distinction of parent's home versus child's home, and investigates multigenerational households in a random sample of adults in Hamilton, Ontario. In a qualitative analysis, the two types of multigenerational households are shown to be related to different family life course phases and to differ on a number of dimensions, including family size, age, effectual and consensual solidarity, income, marital status, and gender. The analysis indicates the need for future research on multigenerational households to explicitly differentiate between the two types of households examined in this paper.

The coincidence of retirement and aging, experienced in the later stages of life, can trigger a recurrence of the bipolar tensions of earlier stages described in Erikson’s model of personality development. The model provides a means for retirees to understand the ground from which their present identity has developed and how the psychodynamics of previous life stages can be reactivated by the onset of retirement and aging. Stages of the model are illustrated by experiences within the context of retirement. The utility of Erikson’s model is suggested as a means of enhancing self-awareness, self-understanding, and adjustment to retirement.

Meaningful social engagement in everyday activities can enhance resident quality of life in nursing homes. In this article, we draw on data collected in a multidisciplinary, international study exploring promising practices in long-term care homes across Canada, Norway, and Germany, to investigate conditions that either allow for or create barriers to residents’ social participation. Within a feminist political economy framework using a team-based rapid ethnography approach, observations and in-depth interviews were conducted with management, staff, volunteers, students, families, and residents. We argue that the conditions of work are the conditions of care. Such conditions as care home location, building layout, staffing levels, and work organization, as well as governing regulations, influence if and how residents can and do engage in meaningful everyday social life in/outside the nursing home. The presence of promising conditions that facilitate resident social participation, particularly those promoting flexibility and choice for residents, directly impacts their overall health and well-being.

This study examined the socioeconomic pathways linking partnership status to physical functioning, assessed using objective measures of late life physical functioning, including peak flow and grip strength. Using Wave 4 of the Survey of Health, Ageing and Retirement in Europe (SHARE), we ran multilevel models to examine the relationship between partnership status and physical function in late life, adjusting for social-network characteristics, socioeconomic factors, and health behaviours. We found a robust relationship between partnership status and physical function. Incorporating social-network characteristics, socioeconomic factors, and health behaviours showed independent robust relationships with physical function. Co-variates attenuated the impact of cohabitation, separation, and widowhood on physical function; robust effects were found for singlehood and divorce. Sex-segregated analyses suggest that associations between cohabitation, singlehood, divorce, and widowhood were larger for men than for women. Results suggest that social ties are important to improved physical function.

In this multiple-case study, we engaged directors of care of Ontario long-term care (LTC) homes in semi-structured interviews designed to increase our understanding of the influence exerted by organizational and extra-organizational factors on two key aspects of organizational performance: operational efficiency and quality of care. We also examined the influence of these factors on the relationship between efficiency and quality. Through a review of the health services and organization and management literatures, four broad factors identified a priori as influential for one or both performance outcomes were used to guide our data collection: staff characteristics, facility characteristics, extra-organizational influences, and the function of volunteers. Our findings suggest that while both high efficiency and high quality of care are achievable, there are aspects of a home’s operations and realities associated with the LTC sector in Ontario that can make achieving both, simultaneously, exceedingly challenging.

Cette étude a examiné un échantillon de 228 personnes âgées en réhabilitation après chirurgie pour remplacement d’une articulation; elle a porté sur la relation entre une adaptation proactive, l’humeur et les résultats psychologiques et fonctionnels. Faire face proactivement c’est une façon de réagir qui est axée sur les objectifs et nécessite qu’on affront les facteurs qui incitent le stress comme un défi plutôt qu’une menace. Selon notre hypothèse, les personnes âgées qui adoptent des stratégies proactives d’adaptation subiraient une amélioration du fonctionnement physique et psychologique apres la réadaptation et connaîtraient une humeur positive. Le modèle proposé a trouvé du support, par lequel une expérience de vigueur a été trouvé à la médiation de l’effet d’une adaptation proactive sur les résultats. Les implications théoriques et pratiques des résultats de recherche sont discutées dans le contexte des interventions qui encourageraient les personnes âgées à faire face proactivement.

Developments in information technology and the ongoing restructuring of health services to increase provision in community settings militate in favour of a streamlining of communications and the exchange of information about patients among health and social care providers. Yet the principles of confidentiality and privacy appear to inhibit this process. In order to explore the practical, ethical, and legal imperatives attendant upon personal health information exchange, we conducted a series of interviews with professional care providers, persons with early-stage dementia, and their family caregivers. The findings indicate some degree of discordance. Professionals reported valuing disclosure both to colleagues and family caregivers on the basis of its being in the patients' best interests. Patients also valued inter-professional exchange, but sought strong control over disclosure to family members. Family caregivers valued being kept informed of the patient's condition, even without the latter's consent. Implications for research and policy are discussed.

Purpose: This study estimates the prevalence of problems with transportation in a sample of community-dwelling seniors residing in an urban setting and investigates the role that gender plays in the ability of seniors to remain mobile in their communities.

Design and Methods: Data collected as part of a study assessing the prevalence and consequences of unmet needs for community-based services in a random sample of 839 elderly aged 75 years and older were employed in bivariate and multivariable analyses.

Results: The prevalence of problems with transportation was 23 per cent, with 33 per cent of females and 10 per cent of males categorized as having problems with transportation. Of those subjects categorized as having problems with transportation, 88 per cent were women. In addition to being predominantly women, those who reported problems with transportation were older, in poorer health, and had lower income and income satisfaction.

Implications: Problems with transportation are an important issue facing seniors; women, in particular. These results highlight the differences in aging as experienced by women and men with respect to social effects, needs, and the significance attached to the experience.

Family caregivers are an integral and increasingly overburdened part of the health care system. There is a gap between what research evidence shows is beneficial to caregivers and what is actually provided. Using an integrated knowledge translation approach, a stakeholder meeting was held among researchers, family caregivers, caregiver associations, clinicians, health care administrators, and policy makers. The objectives of the meeting were to review current research evidence and conduct multi-stakeholder dialogue on the potential gaps, facilitators, and barriers to the provision of caregiver supports. A two-day meeting was attended by 123 individuals. Three target populations of family caregivers were identified for discussion: caregivers of seniors with dementia, caregivers in end-of-life care, and caregivers of frail seniors with complex health needs. The results of this meeting can and are being used to inform the development of implementation research endeavours and policies targeted at providing evidence-informed caregiver supports.

A stratified random sample of 400 community-dwelling older persons were asked whether they like or dislike anything about being their age and if they have any worries about growing older. The results of multivariate analysis show significant relationships between the dependent variables and several respondent charaderistics including age, subjective health, expectations of older age, and gender. A negative view of aging appears to be associated with experiencing age-related declines. Overall, despite some diversity, the respondents tend to hold a positive view of old age that is coupled with a realistic appreciation of the shortcomings this stage of life may bring.

Discontinuity of grandparent-grandchild relationships has been largely overlooked in social science research, despite the fact that the salience of the grandparent grandchild attachment bond is the subject of considerable discussion. This article reports the results of an exploratory study of grandparent-grandchild contact loss, by means of a survey of the core membership of five “grandparent rights” groups across Canada. As reflected in the particular concerns of this membership, there are four primary circumstances associated with grandparent loss of contact with grandchildren: parental divorce, conflict with both parents, death of adult child, and stepparent adoption following remarriage. Grandparents whose adult children are noncustodial parents (mostly paternal grandparents) are at high risk for contactloss, and adult children-in-law appear to be the primary mediators in the ongoing grandparent-grandchild relationship. Disrupted grandchild access is seen as having profound negative consequences for grandparents, and this has important implications for socio-legal policy and therapeutic practice.

The move towards early retirement as witnessed over the last few years in Europe comes from social assistance mechanisms, other than old age security and is not due to a simple earlier retirement age.

Two systems have been used primarily to help older workers. These are disability and employment insurance. “Early retirement” mechanisms have also helped these active or unemployed workers retire, by means of allocating resources.

The social security structure in European countries has remained essentially unchanged with the risks and logistics of management inextricably intertwined. Moreover, these new forms of transition between activity and retirement reveal the reorganization underway in all matters related to aging workers. One of the implications of the massive movement towards early retirement has been that the tripartite life course in which important social stages within the life course are marked by milestones (e.g. school age, retirement age …) is disappearing. It is gradually being replaced with new flexibility for managing the end of the life course. This type of evolution would require us to rethink the social protection system in the sense of reducing the distinct ternary division in the life course. In this perspective, the very concepts of retirement and social transfers for definitive inactivity are no longer relevant.

Longitudinal studies are examined with special reference to the Ontario Longitudinal Study of Aging (LSA). The specific aspects discussed are the representativeness of the sample, attrition rates, end-points, and associations, particularly the associations with age. It is concluded that, in comparison with other longitudinal studies, both in Canada and the U.S., the LSA represents a good source of longitudinal data and therefore lends itself to analyses which may be used to investigate factors expected to be important in understanding various aspects of the aging process.
