It all still seems relatively new and has passed in the blink of an eye, but it is 4 years since I assumed responsibility for Acta Neuropsychiatrica. Time has flown, but during this period with the help of many friends and colleagues, I am pleased to say that the journal has managed to create a unique environment in which research and the diverse expression of opinion have flourished. Some sections of the journal have met with initial scepticism and then acceptance, others still raise eyebrows and invoke comments, this is healthy but I feel it is time to review our current journal structure and publication format. To this end I welcome any input from readers and those on the editorial and advisory boards. The journal will post a feedback form asking for ratings of the journal and its various sections and then publish this information in a future editorial.

In order to keep in touch, it is important to remain in touch, and hence this initiative to find out ‘what we are doing well’, ‘what can be done better’ and ‘what changes need to be made’. In addition to the survey, which we will post electronically and on the website, I would value any informal feedback or comments from those familiar with the journal.

Finally, in this issue we have a number of interesting articles that capture this sense of motion with original reports, special sections and commentaries on blepharospasm, involuntary movements and tremor (Reference Dias, Doyle, Kummer, Cardoso, Fontenelle and Teixeira1Reference Kargiotis, Papathanasopoulos, Terzis, Yiannopoulos and Andonopoulos5).
